An unusual case of herniation of small bowel through an iatrogenic defect of the falciform ligament

Abstract: Internal herniation through a congenital defect in the falciform ligament is extremely rare. We report an unusual observation of small bowel obstruction through an iatrogenic defect of the falciform ligament. Prompt diagnosis was made by helical CT, permitting a rapid surgical procedure to preserve the viability of the obstructed segment.

“…The first case of falciform hernia was described in 1929 and each decades is characterized by an increasing number of new cases, as such as there is an ever-increasing rate of cases done laparoscopically. This evidence suggests logically the association between the two events [10], [15], [16]. In the above-described case, the defect did not appear to be iatrogenic because the patient had never undergone abdominal surgical procedures.…”

“…Le contenu herniaire est le plus souvent du grê le, comme dans notre observation ; il peut s'agir plus rarement de l'é piploon ou du cô lon [5]. Dans la majorité des cas, le tableau est celui d'une occlusion grê lique sur un abdomen vierge ou encore chez un patient aux anté cé dents de chirurgie sur l'é tage sus mé socolique [2]. Le diagnostic de hernie interne à travers le ligament falciforme peut ê tre é voqué sur les donné es des examens morphologiques.…”

“…9 The demonstration of proximal small bowel dilatation with particular prominence at the hepatic hilum extending to the falciform fissure and collapsed bowel loops to the left of the fissure is strongly suggestive of a herniation through the falciform ligament. 3 Laparoscopic cholecystectomy is now considered the gold standard operation for patients with gallstone disease and is the most common laparoscopic operation performed. With the advent of laparoscopic cholecystectomy, there has been a significant change in the surgical management of gallbladder disease with an increased predilection to recommend elective cholecystectomy.…”

“…Currently, only twocases of bowel obstruction secondary to iatrogenic falciform ligament apertures have been reported in the literature. 2,3 In the era of laparoscopic surgery, we present an unusual case of small bowel obstruction following laparoscopic cholecystectomy.…”
