An Unusual Case of Carcinoma Erysipelatoides Originating from Gastric Adenocarcinoma

Abstract: Carcinoma erysipelatoides, also known as inflammatory metastatic carcinoma, is a rare form of cutaneous metastasis. It is most commonly caused by breast carcinoma, but rarely associated with gastric adenocarcinoma. Herein, we report an unusual case of carcinoma erysipelatoides in a 55 year-old female who had been diagnosed with signet ring cell carcinoma of the stomach metastasizing to ovaries.

“…However, some skin cancers may also contain SRCs (squamous cell carcinoma, basal cell carcinoma, primary cutaneous SRC carcinoma of eccrine or apocrine origin, melanocytic tumours (nevi or melanomas), liposarcomas, hydradenomas, cylindromas, lymphomas, and the atrophic variant of mycosis fungoides, which could constitute potential alternative diagnosis. 3,4 Moreover, the particular immunochemistry profile found from the skin biopsy was also identified in other carcinomas such as cholangiocarcinoma, 5 malignant mesothelioma, 5,6 breast carcinoma, 5,7 ovarian carcinoma 5 and endometrial adenocarcinoma 5,8 which could be differentiated by their own specific immunochemistry markers.…”

“…2 The same location has been exceptionally described in 1984, namely connubial melanoma of the choroid. 3 Many other descriptions of melanoma in spouses are reported. 4,5 Because of them, our real question is whether the reason of the melanoma is coincidental or the expression of exposure to the same risk factors.…”

“…Signet ring cells are most frequently associated with stomach cancer but can arise from other tissues including the bladder, breast, colon, and lung. 3 Thus, the presence of signet ring cells in a skin biopsy specimen suggests Figure 1 (a, b) Clinical manifestation of metastatic gastric carcinoma in a 73-year-old woman. Pruritic, well-demarcated, erythematous to brownish, slightly indurated, edematous plaque is apparent on the left side of the neck, chest wall and left arm metastatic SRCC from the gastrointestinal tract, bladder, breast, or lung.…”

“…However, cutaneous neoplasms that may contain signet ring cells, including non-Hodgkin's lymphoma, primary cutaneous SRCC of eccrine or apocrine origin, primary cutaneous signet ring basal cell carcinoma, and squamous cell carcinoma, must be excluded. 3 The diagnosis of cutaneous metastasis is important because the disease has a poor prognosis. Therefore, a dermatologist examining new unusual skin lesions in a patient with a history of malignancy should consider the possibility of unusual metastases to the skin and subsequently obtain a skin biopsy from the suspicious lesion.…”
