Purpose Ectopic pregnancy (EP) is rare among patients having received tubal ligation; in these cases, a fertilized ovum is implanted in the proximal tube stump. Distal tubal EP cases in patients having undergone ipsilateral tubal ligation and who have relatively intact contralateral adnexa are extremely rare. Here, we report a case of pregnancy in the distal stump of the ipsilateral fallopian tube following fallopian tubal isthmus ligation. Case Presentation A 28-year-old woman was admitted after having experienced lower abdominal pain for 10 days and amenorrhea for 1 week and receiving a transvaginal color ultrasonography finding of a heterogeneous echo (2.1 × 1.2 × 1.4 cm) near the left ovary. The patient had a medical history of transvaginal left tubal ligation under single-port laparoscopy for a left hydrosalpinx. Postoperatively, the patient had undergone in vitro fertilization for assisted reproduction. After ovum retrieval, whole-embryo cryopreservation had been performed due to the ovarian hyperstimulation syndrome trend. This pregnancy was natural after embryo cryopreservation. After the patient was admitted, laparoscopic exploration revealed an elevated ampulla in the distal stump of the left fallopian tube. Left salpingectomy with removal of the ectopic pregnancy in the distal remnant tube was performed under the guidance of transvaginal single-port laparoscopy. Serum β-human chorionic gonadotropin levels gradually decreased. Thereafter, the patient underwent two cycles of frozen embryo transfer, both of which resulted in chemical pregnancies. Conclusion This case suggests that gynecologists should pay attention to the possibility of fallopian tube ectopic pregnancy in the distal tubal segment post-tubal ligation.

Objective To report a rare case of spontaneous ectopic pregnancy (EP) after partial salpingectomy treated by laparoscopy. Case Report A 30-year-old gravida 1 para 0 woman with a history of left adnexectomy, due to left ovarian torsion 13 years ago, was referred to our hospital. She had experienced lower abdominal pain for 1 day and amenorrhea for 1 week. Transvaginal ultrasonography did not reveal an intrauterine pregnancy, but showed a suspected extrauterine gestational sac on the left adnexal area. The patient was diagnosed with ectopic pregnancy, and laparoscopy was performed. During the operation, we found a gestational sac on the left fallopian tube stump. Conclusion The most significant risk factors for ectopic pregnancy (EP) are previous procedures and conditions that cause tubal injury. Therefore, it is necessary to be alert to the occurrence of the disease even if there is a history of tubal resection and ensure no part of the fallopian tube is left behind during removal.

Objetivo: descrever o perfil de mulheres com gravidez ectópica (GE) atendidas em um hospital público do Rio Grande do Sul (RS). Método: trata-se de uma pesquisa transversal, descritiva, com abordagem quantitativa retrospectiva. O estudo foi realizado em um serviço de ginecologia de um hospital público de grande porte. A amostra constituiu-se da análise de 313 prontuários de mulheres com diagnóstico de gestação ectópica, no período de 01 de janeiro de 2014 a 31 de dezembro de 2019. Resultados: do total de 313 prontuários de mulheres com GE analisados, a mediana de idade foi de 28 anos, 239 eram brancas, 269 eram solteiras e 166 possuíam ensino fundamental. A mediana da idade gestacional foi de 49 dias, sendo que 229 apresentaram gestações prévias. Não apresentaram doenças prévias 268 mulheres, entretanto, 167 apresentaram algum fator de risco, sendo os mais frequentes: aborto espontâneo (22,0%), tabagismo (19,8%), idade avançada (14,7%) e gestação ectópica prévia (12,1%). A salpingectomia (62,6%), o metotrexato (10,2%) e o tratamento conservador (15,0%) foram os principais tratamentos adotados, sendo a trompa direita (47,9%) e a trompa esquerda (41,2%) as principais localizações da gravidez. Conclusão: estratégias de qualificação da atenção à saúde das mulheres em idade reprodutiva podem ser implementadas, bem como, o estabelecimento de protocolos clínicos e obstétricos para investigação e diagnóstico precoce da GE.
