An Outbreak of Multiresistant Acinetobacter baumanii in a University Hospital in São Paulo, Brazil

Abstract: A case-control (46 cases, 23 controls) study was done to determine risk factors for an outbreak of a multiresistant Acinetobacter baumanii (only susceptible to colistin) in a university hospital. The use of antecedent antibacterials and intubation were independent risk factors. No common source was found. With control measures, the outbreak resolved gradually.

“…Numerous outbreaks of nosocomial infections caused by multiresistant A. baumannii have been reported in various countries, including Brazil (Oliveira et al 1993, Bergogne-Bérézin & Towner 1996, Levin et al 1996, Vaz et al 1996. Important risk factors are involved in the urinary tract infections, which occur mostly in elderly debilitated patients confined to ICU, and with permanent urinary catheters.…”

“…This gene is intrinsic to the species and has been identifi ed in a wide collection of isolates of the A. calcoaceticus-A. baumannii complex collected from various geographic areas (13) (28) (29) (31) (32) . Thus, phenotypically identifi ed isolates that possess the bla OXA-51-like gene were considered members of the complex in the present study.…”

“…baumannii complex has been responsible for nosocomial infection outbreaks in Europe since 1980, mainly in England, France, Germany, Italy, Spain, and the Netherlands (16) . In Brazilian hospitals, infections have been reported since 1996, mainly at ICUs (17) . Carbapenems became the primary therapeutic options in various hospitals, but the continued use of these antimicrobials has been compromised by the emergence of novel resistance mechanisms (1) .…”
