Het obstructief slaapapneusyndroom is een slaapstoornis die wordt gekarakteriseerd door veelvuldig optredende luchtweg obstructies tijdens de slaap. Bij kinderen gaat de slaapstoornis gepaard met medische en ontwikkelingsgerelateerde aandoeningen en afwijkingen, zoals groeistoornissen, cardiovasculaire complicaties en neurocognitieve problemen. Onlangs is de landelijke multidisciplinaire richtlijn voor de diagnostiek en behandeling van het obstructief slaapapneusyndroom bij kinderen opgesteld. In deze richtlijn wordt adenotonsillectomie als de behandeling van eerste keus beschouwd. Bij kinderen met ernstige obstructieve slaapapneu als gevolg van middengezichtshypoplasie of mandibulaire hypoplasie kan distractie worden overwogen. De richtlijn vermeldt een bescheiden rol voor orthodontische behandeling van de dysgnathie. Met een mandibulair repositieapparaat en snelle sutuurexpansie kan het volume van de bovenste luchtweg worden vergroot. Volgens de nieuwe richtlijn kan orthodontische behandeling als secundaire optie bij de behandeling van het obstructief slaapapneusyndroom bij kinderen met bepaalde afwijkende kaakrelaties worden overwogen. Wat is nieuw?In 2013 is een landelijk richtlijn voor de diagnostiek en behandeling van OSAS bij kinderen verschenen. In deze richtlijn komen ook orthodontische behandelopties aan de orde. PraktijktoepassingVolgens de nieuwe richtlijn kan behandeling met een mandibulair repositieapparaat en 'rapid maxillary expansion' door orthodontisten als secundaire behandeloptie bij kinderen met OSAS en bepaalde dysgnathieën worden toegepast. InleidingHet obstructief slaapapneusyndroom (OSAS) is een slaapgerelateerde ademhalingsstoornis, die wordt gekenmerkt door herhaalde episodes van hogere luchtwegobstructie tijdens de slaap. Deze perioden gaan gewoonlijk gepaard met zuurstofdaling in het bloed. De obstructie wordt veroorzaakt doordat een te nauwe en/of te slappe farynx tijdens de slaap collabeert. Hierdoor treedt een onderbreking van de luchtstroom op (apneu). Pas na een ontwaakreactie ('arousal') treedt herstel van de luchtwegdoorgankelijkheid op en wordt de ademhaling weer hervat. Het obstructief slaapapneusyndroom gaat doorgaans gepaard met snurken en bewegingsonrust tijdens de slaap. De frequente ontwaakreacties verstoren het normale slaappatroon. Bij kinderen kan OSAS leiden tot slaperigheid overdag, gedrags-en concentratieproblemen, slechte schoolprestaties, depressiviteit, groeistoornissen, hypertensie en cor pulmonale. Een te nauwe en/of te slappe farynx kan het gevolg zijn van adenotonsillaire hypertrofie, obesitas, musculaire hypotonie en craniofaciale afwijkingen.In 2009 is de landelijke richtlijn voor OSAS bij volwassenen volledig herzien (Kwaliteitsinstituut voor de Gezondheidszorg CBO, 2009 CBO (CBO, 2013). Recent research suggests supine positioning for sleeping infants is the safest position to prevent breathing related difficulties; however doing so can significantly increase obstruction in apneic infants resulting in decreased sleep quality. We implemented a multi-component treatment package compromised of antecedent interventions and parentmediated social reinforcement to increase lateral positioning in a premature infant with obstructive sleep apnea. Results indicate that the intervention increased lateral positioning in the participant by over 80 % in the final phase of the study, indicating efficacy of the intervention. Limitations and suggestions for future research are discussed.
