An Italian multicentre distributed data research network to study the use, effectiveness, and safety of immunosuppressive drugs in transplant patients: Framework and perspectives of the CESIT project

Abstract: The goal of post-transplant immunosuppressive drug therapy is to prevent organ rejection while minimizing drug toxicities. In clinical practice, a multidrug approach is commonly used and involves drugs with different mechanisms of action, including calcineurin inhibitors (CNI) (tacrolimus or cyclosporine), antimetabolite (antimet) (mycophenolate or azathioprine), inhibitors of mechanistic target of rapamycin (mTOR) (sirolimus or everolimus), and/or steroids. Although evidence based on several randomized clinic… Show more

“…Over the last two years, TheShinISS framework has been employed in several large-scale observational studies exploring the association between some exposures and COVID-19 onset/prognosis as well as other drug and vaccinerelated research topics and is currently maintained by a collaborative research network. 1,2,[9][10][11][12][13] The relational scheme of the study databases as well as TheShinISS flow diagram is described in Figure 1. is the author/funder, who has granted medRxiv a license to display the preprint in (which was not certified by peer review) preprint…”

“…Within this subset, there were 2,135 patients aged 18years or older, who had undergone transplantation for the rst time and survived for at least 30 days while using a CNI. Record linkage with the National Transplant Information System was successful, allowing identi cation of approximately 90% of patients using pseudoanonimous information (11). The 1,909 dead donor liver transplants were strati ed into 750 due to cirrhosis (39.3%) and 1,159 for HCC (60.7%).…”

“…Demographic and clinical characteristics of both donors and recipients, which were available nationwide, were linked through a semi-deterministic matching procedure (11). The study cohort was limited to patients residing in the considered regions who survived and had received at least one CNI immunosuppressive dispensation during the 30 days following their discharge (index period).…”
