Nurse-led follow-up of patients after oesophageal or gastric cardia cancer surgery: a randomised trial

Between January 2004 and February 2006, 109 patients after intentionally curative surgery for oesophageal or gastric cardia cancer were randomised to standard follow-up of surgeons at the outpatient clinic (standard follow-up; n =55) or by regular home visits of a specialist nurse (nurse-led follow-up; n =54). Longitudinal data on generic (EuroQuol-5D, European Organization for Research and Treatment of Cancer (EORTC) QLQ-C30) and disease-specific quality of life (EORTC QLQ-OES18), patient satisfaction and costs were collected at baseline and at 6 weeks and 4, 7 and 13 months afterwards. We found largely similar quality-of-life scores in the two follow-up groups over time. At 4 and 7 months, slightly more improvement on the EQ-VAS was noted in the nurse-led compared with the standard follow-up group ( P =0.13 and 0.12, respectively). Small differences were also found in patient satisfaction between the two groups ( P =0.14), with spouses being more satisfied with nurse-led follow-up ( P =0.03). No differences were found in most medical outcomes. However, body weight of patients of the standard follow-up group deteriorated slightly ( P =0.04), whereas body weight of patients of the nurse-led follow-up group remained stable. Medical costs were lower in the nurse-led follow-up group (€2600 vs €3800), however, due to the large variation between patients, this was not statistically significant ( P =0.11). A cost effectiveness acceptability curve showed that the probability of being cost effective for costs per one point gain in general quality-of-life exceeded 90 and 75% after 4 and 13 months of follow-up, respectively. Nurse-led follow-up at home does not adversely affect quality of life or satisfaction of patients compared with standard follow-up by clinicians at the outpatient clinic. This type of care is very likely to be more cost effective than physician-led follow-up.

Abstract:Background: Patients' satisfaction has become an important tool to assess the quality health care services, which is progressively required by accreditation agencies in the checking of quality of hospital care. Aim: evaluate oncology patient satisfaction towards health care services at University Hospital after accreditation. Methods: Quantitative, descriptive study was used. This study was conducted in five units as the medical; surgical (male and female) and gynecology wards at King Abdulaziz University Hospital. It is one of the first Hospitals in the eastern Mediterranean region implement health care accreditation standards. Sample: Total number of non-randomized convenience sample was 123 oncology patients who admitted to the above-mentioned setting. Patients were selected for their oncology diagnosis from each department and who is oriented and conscious included in the study. Data was collected by structured interview questionnaire for measuring patient satisfaction level towards health care services. Results: Studied sample was satisfied by the knowledge and an experience they give about illness; information about medical tests; and information is given about treatment at (mean = 4.67); highly statistically, significant difference's relationship was observed in the total level of patient's satisfaction, and care received from nurses to the oncology patient during his hospital staying and regarding to services care organization at p = .000. Conclusion: there was satisfaction with the information provided by the medical staff about the patient's illness and the course of treatment. This is followed by the time spent with the physician and the interpersonal skills of the physician and nurses. The implication of study: develop the training program for health care providers about effective documentation; guidelines for providers in care for similar patients, and different formal monitoring systems are used by health care providers or regulators.

La evaluación de la calidad de vida es uno de los elementos clave de la atención que se ofrece al paciente oncológico. Este trabajo presenta la línea de investigación que sobre la calidad de vida se ha realizado desde 1992 en los servicios de oncología del Hospital de Navarra. Se trabaja dentro del Grupo de Calidad de Vida de la Organización Europea para la Investigación y Tratamiento del Cáncer -EORTC-en el desarrollo de cuestionarios y en otros proyectos. Se ha coordinado la creación de la escala de la EORTC de información. Se han validado instrumentos de medida de dicho organismo para nuestro país. Se han llevado a cabo estudios de calidad de vida en tumores y en satisfacción de los pacientes con los cuidados recibidos. Medición. absTracTQuality of life assessment is one of the key elements of the care that is offered to cancer patients. The aim of this work is to present the research line on quality of life that has been carried out since 1992 in the Oncology Departments of the Hospital de Navarra. These departments actively collaborate with the European Organisation of Research and Treatment of Cancer -EORTC-Quality of Life Group in creating questionnaires and also in other projects of this group. Our institution has coordinated the development process of the EORTC information module. Different EORTC questionnaires have been validated for use in our country. Quality of life studies have been carried out in the main tumour sites and in other areas, such as patients' satisfaction with care. This research line has a direct benefit on the attention that patients receive.
