During the early stages of the coronavirus disease 2019 (COVID-19) pandemic, EASL and ESCMID published a position paper to provide guidance for physicians involved in the care of patients with chronic liver disease. While some healthcare systems are returning to a more normal routine, many countries and healthcare systems have been, or still are, overwhelmed by the pandemic, which is significantly impacting on the care of these patients. In addition, many studies have been published focusing on how COVID-19 may affect the liver and how pre-existing liver diseases might influence the clinical course of COVID-19. While many aspects remain poorly understood, it has become increasingly evident that pre-existing liver diseases and liver injury during the disease course must be kept in mind when caring for patients with COVID-19. This review should serve as an update on the previous position paper, summarising the evidence for liver disease involvement during COVID-19 and providing recommendations on how to return to routine care wherever possible.

The coronavirus pandemic has changed the priorities of the whole medical society. During the clinical course of COVID-19, it has been observed that hepatic injury occurs in a significant proportion of patients, particularly in those with severe or critical illness. In this literature review, we summarize the most recent studies, which covered the pathophysiology of COVID-19 induced liver injury including; hepatic pathological findings, therapy related liver damage, and the effects of the viral infection on pre-existing liver diseasesin context of the most recent recommendations. Conclusions: This review sheds light on the impact of COVID-19 infection on the liver, as well as the prognostic effect of liver laboratory markers on disease outcome. Temporal variations in liver parameters during disease course as well as different patterns of derangement are depicted. More intensive surveillance and individualized therapeutic approaches should be tailored for immunocompromised patients with advanced liver disease, hepatocellular carcinoma, and liver transplant patients. Despite the limited studies on COVID-19 infected patients with preexisting liver disease, this comprehensive overview provides a perspective on the management of liver disease during COVID-19.

La pandemia por SARS-CoV-2/COVID-19 ha afectado todas las áreas de la atención médica, particularmente la atención de enfermedades crónicas. La enfermedad hepática crónica es altamente prevalente en nuestro país y representa una de las principales causas de mortalidad general. Objetivo: En esta revisión, analizamos la literatura disponible hasta el 5 de junio de 2020 sobre el impacto de SARS-CoV-2/COVID-19 en enfermedades hepáticas y en la actividad de trasplante hepático. Material y métodos: Realizamos una búsqueda en PubMed con los términos «liver» (hígado), «hepatic» (hepático), o «transplant» (trasplante) y «COVID-19» o «SARS-CoV-2» contenidos en el título o el resumen de la categoría «Journal Articles» durante el último año. Los artículos fueron analizados por dos revisores de forma independiente. Resultados: Encontramos 276 resultados, de los cuales 54 artículos cumplieron con los criterios de inclusión. La mayor parte de la información disponible proviene de recomendaciones de expertos o consensos. Los organismos reguladores de donación y trasplante han desarrollado algoritmos específicos para su región con el objetivo de guiar la actividad de trasplantes durante la pandemia. Los programas, en general, han reportado disminución en sus actividades. Los resultados de infección por SARS-CoV-2 en receptores de trasplante hepático no son concluyentes y la evaluación del donante para la infección es ampliamente recomendada. Conclusión: La pandemia por SARS-CoV-2/COVID-19 ha modificado las prácticas de todos los programas de trasplante alrededor del mundo. La comunidad de trasplantes se encuentra frente a un escenario difícil con evidencia científica escasa que guíe la toma de decisiones sobre el futuro y la atención óptima de pacientes con enfermedad hepática terminal en necesidad de trasplante, durante y después de la pandemia.
