An Interleukin-6-Producing Cardiac Myxoma Associated with Mediastinal Lymphadenopathy

Abstract: We report our experience with a patient whose mediastinal lymphadenopathy resolved after resection of a cardiac myxoma that secreted interleukin-6 (IL-6). The patient was a 68-year-old female who complained of nocturnal chest discomfort related to congestive heart failure. An echocardiogram demonstrated a large left atrial mass. A computed tomogram showed not only the left atrial mass but multiple enlarged mediastinal lymph nodes. The serum IL-6 level was markedly elevated at 13.7 pg/ml. After resection of the… Show more

“…As shown in the present report, IL-6 from cardiac myxoma can induce several other conditions such as multiple cerebellar hemorrhages, 12 pseudovasculitis, 14 mediastinal lymphadenopathy, 15 and left ventricular hypertrophy. 16 However, there has been no report of cardiac myxoma with plasma cell infiltration, which suggests CD.…”

“…There are several interesting cases of cardiac myxoma complicated by either mediastinal lymphadenopathy or left ventricular hypertrophy. 51,52) These cases may suggest a clinical role for IL-6 in the heart. Interleukin-6 in heart transplantation: Myocardial dysfunction in the donor heart is a common problem and is so severe as to preclude its use for transplantation in 20% of cases.…”

“…Of note, expression of IL-6 also has been observed in some cases of this neoplasm (20,21). EMH in cardiac myxomas thus also could be related to local production of IL-6.…”
