An interesting case of adenoviral hepatitis in a cardiac transplant recipient

Abstract: Adenoviral infections are commonly described in pediatric transplant populations. However, much less information is available regarding the incidence of infection and clinical spectrum of disease in adult transplant recipients. Moreover, this infection usually manifests as involvement of the transplanted organ in one pathologic form or the other, in addition to other systemic manifestations. We present a case of adenoviral infection of a nontransplanted organ in a solid organ transplant recipient.

“…121 124 AdV infections have been seen principally in pediatric heart transplant recipients (HTRs) 203 226 ; in adult HTRs, serious AdV infections are limited primarily to case reports. 227 228 229 In pediatric HTRs, AdV has been observed in myocardial biopsies with or without necrosis or inflammation. 217 226 The presence of AdV in myocardial tissues increases the risk for graft loss and posttransplant coronary artery disease.…”

“…8 Shirali et al 16 isolated mainly the serotype 2 from myocardial biopsies of their heart transplant recipients that developed adverse cardiac events. Few reports of adenovirus pneumonia, 18,19 hepatitis, 20 and disseminated disease have been published. 21 In heart transplantation, besides the invasive adenovirus disease which is similar to other solid organ transplants, the allograft can be affected indirectly.…”
