An Intact Dissecting Baker’s Cyst Mimicking Recurrent Deep Vein Thrombosis

Abstract: We report a case of a 75-year-old female with a history of acute deep vein thrombosis (DVT) 6 years ago who presented with unilateral calf swelling and pain. D-dimer was normal, and compression ultrasound revealed findings typical of DVT, including an incompressible dilated and hypoechoic peroneal vein. Despite 4 months of anticoagulation for supposed recurrent DVT, pain symptoms persisted and repeat D-dimer and compression ultrasound were unchanged. A magnetic resonance imaging scan to investigate the leg dem… Show more

“…Finally, the procedure can relieve discomfort in the popliteal fossa region (16). The popliteal cyst symptoms may manifest as calf pain, positive Homan's sign, swelling, infection, compressive neuropathy, posterior compartment syndrome, and other findings mimicking the calf thrombophlebitis or vein thrombosis, especially if they ruptured (17)(18)(19)(20)(21)(22)(23). Therefore, cystectomy may be required, especially when the size of the popliteal cyst is greater than the baseline of 13.4 cm 2 for the patient seeking TKA (9).…”

“…Lying between several surrounding cysts, the main cyst expanded in a posterosuperior direction and compressed the popliteal artery despite its modest size. 5 , 6 In addition, the polycystic nature might have led to limited efficacy of needle aspiration. Thus, surgical resection was inevitable to relieve the pain and to avoid severe limb ischemia for this unusual Baker cyst with arterial flow disturbance.…”
