An Expert Consensus Statement on the Management of Large Chondral and Osteochondral Defects in the Patellofemoral Joint

Abstract: Background: Cartilage lesions of the patellofemoral joint constitute a frequent abnormality. Patellofemoral conditions are challenging to treat because of complex biomechanics and morphology. Purpose: To develop a consensus statement on the functional anatomy, indications, donor graft considerations, surgical treatment, and rehabilitation for the management of large chondral and osteochondral defects in the patellofemoral joint using a modified Delphi technique. Study Design: Consensus statement. Methods: A wo… Show more

“…An expert panel on cartilage restoration deemed concurrent anatomic abnormalities to be of utmost importance to be corrected during the cartilage restoration procedure. 26 The biomechanical literature has demonstrated that unloading procedures can reduce joint surface pressures by up to 30%. [27][28][29] To this note, Black et al 28 also remark on the importance of controlling tibial slope to prevent unwanted loads to the tibial plateau.…”

“… 28 , 29 An expert panel on cartilage restoration deemed concurrent correction of anatomic abnormalities to be of paramount importance during the cartilage restoration procedure. 30 The biomechanical literature has demonstrated that unloading procedures, such as tibial tubercle osteotomy (TTO) can reduce joint surface pressures by up to 30%. 28 , 29 Literature examining unipolar chondral defects have demonstrated improved reported outcomes when realignment osteotomy was performed.…”

“…Die autologe Chondrozytentransplantation (ACT) stellt den Goldstandard der Versorgung von vollschichtigen symptomatischen patellofemoralen Knorpeldefekten dar [27]. Multicenterdaten zeigen gute bis exzellente Ergebnisse in 80 % der Fälle sowohl in klinischen als auch radiologischen Erhebungen im Langzeit-Follow up über 10 Jahre [28][29][30].…”
