Aim:The aim of the study was to compare and evaluate the clinical efficacy of a microfilled pit and fissure sealant and a nanofilled pit and fissure sealant at 3, 6, and 12 months of interval. Materials and methods: Samples consisting of 55 healthy 8-to 12-year-old children with deep pits and fissures in mandibular first permanent molars were selected for the study. It was a split mouth design and randomized clinical trial. A total of 110 mandibular first molars were divided into two groups of 55 each: group I Fissurit FX sealant and group II Grandioseal nanofilled fissure sealant. The sealed teeth were clinically evaluated at 3, 6, and 12 months of interval to assess marginal adaptation, sealant retention, fissure caries development, roughness of sealant surface, and change of color around the sealant. Results: The results showed that both Fissurit FX and Grandioseal pit and fissure sealants were effective in preventing dental caries. Marginal adaptation was significantly better with Fissurit FX when compared to Grandioseal pit and fissure sealant. There was no difference in sealant retention between the two groups. The surface roughness of Fissurit FX was high when compared to that of Grandioseal. Statistical analysis was done using the Chi-squared test for intra-group comparison and Fisher's exact test for inter-group comparison. Results were considered statistically significant if p ≤ 0.05. Conclusion: Fissurit FX and Grandioseal pit and fissure sealants provided similar caries preventive effects and there was no difference in retention of sealants over a period of 1 year. However, surface roughness was better with Grandioseal fissure sealants. Clinical significance: This study is significant because there is limited evidence about the efficacy of nanofilled pit and fissure sealants in vivo . It will also provide dental practitioners an insight into the clinical efficacy of nanofilled pits and fissure sealant when compared to micro-filled sealant enabling them to make the right choice for the betterment of their dental practice.

Systematic Review of Conservative Operative Caries Management Strategies

The relationship between cavity preparation extension and restoration longevity is examined through a systemic review of the available evidence on specific conservative, operative caries‐management strategies. Evidence tables are provided for three specific techniques in the permanent dentition: 1) the proximal “tunnel” restoration, 2) the proximal “box‐only” restoration, and 3) the preventive resin restoration. In the primary dentition, the clinical trials involving the proximal box‐only restoration, most of which involved glass‐ionomer materials, are reviewed. In the permanent dentition, the evidence reveals low effectiveness for “tunnel” restorations, limited but supportive results for proximal‐only restorations, and generally favorable outcomes for the occlusal composite resin‐sealant restoration. The weak link in the latter is the overlying fissure sealant, which requires adequate ongoing maintenance. Conservative operative strategies in the primary dentition have not been uniformly successful, and deleterious material effects dominate restoration performance. This systematic review concludes that operative conservatism per se does not guarantee increased restoration longevity and that all restorations are vulnerable to caries recurrence, material failures, and technical deficiencies. The more successful conservative strategies are expected to enhance tooth longevity, provided concomitant caries control is effective.

Best Practices for Dental Sealants in Community Service‐Learning

Community service-learning (CSL) in predoctoral dental education might be an effective tool for increasing sealant use by dentists-thus benefitting underserved children while facilitating students' learning of a clinical procedure in a real-life setting. This study reviewed the scientific literature on this topic in order to 1) evaluate the reasons for low sealant use among dentists, 2) consider important aspects of sealant use in community settings, and 3) identify best practices to use as guidelines for CSL regarding sealant use. As background, the MEDLINE database was searched with the key words "dental sealants" for human and laboratory studies in the English language. A total of 205 relevant articles were identified and overviewed. We found that the low use of sealants relate to dentists' orientation toward restorations rather than prevention, distrust in sealant treatment, lack of confidence in caries risk assessment, and concern about sealing over caries. The aspects to consider in the CSL projects are acquisition of knowledge and necessary skills of operators, cost-benefit approach to sealant placement, and meticulous sealant placement procedures, including the necessity for a short-term recall.
