An Envenoming Syndrome from Massive Vespa Stings Induces Multiple Organ Failure

Abstract: Envenoming syndrome is a systemic reaction induced by inoculation of large volumes of Hymenoptera venom. The clinical manifestations range from skin allergic reactions to multiple organ failure. Vespid venom-induced toxic reactions and anaphylaxis are the most common lethal mechanism of death, involving acute respiratory failure, acute liver failure, rhabdomyolysis, acute kidney injury, and severe coagulopathy. Multiple organ failure as a consequence of severe venom toxicity is a rare but dangerous complicatio… Show more

“…Hyaluronidase hydrolyzes the hyaluronic acid between cells, probably facilitating a smooth entry of toxins into body tissues, possibly exacerbating local inflammation. 6 In this study, the patient had normal platelets on admission, which decreased to 42 × 10 9 ·L −1 on Day 8 of admission. A report in India demonstrated that thrombocytopenia was found in patients stung by wasps, and the authors suggested that thrombocytopenia is a direct result of wasp venom’s effect on platelets.…”

“…A decrease in hepatic synthetic functions can also be observed, although the coagulopathy is contributed by DIC. A significant proportion will progress to acute fulminant hepatic failure [51] while other patterns of hepatic injury such as Reye syndrome have also been observed [52].…”
