An atypical pleomorphic lipomatous tumor arising on the cheek

Abstract: Pathologists and dermatopathologists commonly encounter tumors with adipocyte differentiation. Most are of minimal clinical significance. Those exhibiting atypical spindle cell morphology have been reported but the terminology for such neoplasms is unsettled. Tumors with both spindle cell and pleomorphic morphology are rare, with equally unsettled descriptive nomenclature. Recently, a series of such tumors, termed atypical pleomorphic lipomatous tumor, has been published. They resided in the deep soft tissue o… Show more

“…I have read with a lot of interest the recent article of Boyd 1 reporting on an atypical pleomorphic lipomatous tumor arising on the cheek and I would like to make a few additional remarks. In my view, purely intradermal atypical pleomorphic lipomatous tumors (also called “atypical spindle cell/pleomorphic lipomatous tumors” (ASPLT) to emphasize the major overlap with the recently characterized atypical spindle cell lipomatous tumors, which I believe to belong to the same clinicopathologic and molecular spectrum 2 ) are extremely uncommon, and most of the cases of purely intradermal adipocytic lesions with spindle/pleomorphic cell features I have seen in consultation, were found to be rather compatible with intradermal spindle cell/pleomorphic lipomas.…”

“…Abdominal wall and groin areas are infrequent locations. Literature search [2][3][4][5][6][7][9][10][11][12][13][14][15][16] shows that Bahadır B et al [2] and Anderson WJ et al [9] each described a case in the groin region. Also, two more cases were described on the abdominal wall [2] and Mariño-Enriquez A et al [6] described 15 cases on the trunk, not specifying the affected regions.…”

“…The real incidence of atypical pleomorphic lipomatous tumor is difficult to assess, as most of our knowledge of this very recently described entity is based on small series or single case reports (Creytens et al 2017(Creytens et al , 2019Bahadır et al 2018;Boyd 2019).…”

“…Atypical pleomorphic lipomatous tumors arise in the subcutis slightly more frequently than in deep (subfascial) somatic soft tissues and only occasionally arise in purely intradermal, intracavitary, or visceral locations. The anatomic distribution is wide, predominating in the limbs and limb girdles (Creytens et al 2017, Mariño-Enriquez et al 2017Bahadır et al 2018;Boyd 2019).…”
