An approach to managing non-melanoma skin cancer of the nose with mucosal invasion: our experience

Abstract: Basal cell carcinoma was the most common lesion (75%), followed by squamous cell carcinoma (25%). Ultrasonography excluded lymphatic involvement for SCC. Before final reconstruction, extemporary histological examination revealed the presence of tumour cells in three patients. After tumour extirpation, these patients were resubmitted to a new follow-up period before reconstruction. No recurrences were observed after definitive nasal reconstruction in all patients during the 5-year follow-up.

“…NMSCs are the most frequent tumours of the skin, and their incidence is higher in sun‐exposed areas. In addition, these tumours often tend to recur, even after wide excisions: SCCs’ recurrence reaches 54%, while BCCs recur in up to 35% of cases . High recurrence rates can be explained by intrinsic NMSC malignancy and by the fact that a conservative approach with narrow excision is applied in order to avoid cosmetic deformity …”

“…In the head region, the nose is the most common site where NMSCs appear . Other areas are the forehead, the scalp, the ear, and infraorbital region as a consequence of the chronic exposure to sun damage.…”

“…This cautious approach follows the rule of halting the reconstruction until confirmation of clear margins on histological examination 10. Moreover, total and subtotal nasal reconstructions are multistaged procedures that can jeopardise a potentially good outcome if performed in a non-radical field.…”
