An Antibiotic Order Form Intervention Does Not Improve or Reduce Vancomycin Use

Abstract: Inappropriate vancomycin use and vancomycin use overall increased after the introduction of an AOF. An AOF intervention did not have its intended effect of improving and reducing vancomycin use.
