“…The RIA has been reported as a source of bone graft with supportive experimental [17] and clinical studies [5,[7][8][9] and case reports [6,[20][21][22][23]. Direct grafting of harvested RIA bone fragments, in addition to 'autografting' alone has been shown to improve stiffness (1.21 Nm compared with 0.48 Nm; P=0.029) and strength (14.09 Nm compared with 6.94 Nm; P=0.029) in a sheep model [19].…”

“…Several investigators noted that the aspirate from the reaming system was laden with bioactive substrates (eg, osteoblasts) and growth factors (eg, bone morphogenetic protein-2 [BMP-2]) after performing cell cultures and flow cytometry [8,9,18,22,25,26]. The RIA system has been used clinically to procure bone graft from the femur; however, few studies describe its clinical use [12,13,15,24].…”

“…Factors include amount of pain, infection rate, amount of bone graft, and ease of the procedure. Intramedullary bone graft harvest with the Reamer-Irrigator-Aspirator (RIA) system (Synthes Inc., USA, West Chester, PA) was developed as an alternative to common bone graft options such as iliac crest bone graft and synthetic bone substitutes [4,21,25,33,44]. Th e origin of this procedure can be traced to Stürmer at al.…”

“…Despite these developments, autogenous bone graft can still be considered the gold standard in defect reconstruction because of its intrinsic osteogenetic activity [1,8,9,15,24,33,39,46,51]. Th e use of collected intramedullary reaming debris has been investigated in several trials and was found to be safe and comparable to other sources of bone graft [4,8,13,14,19,20,33,36,37,42].…”
