An Algorithmic Approach to Surgical Decision Making in Acetabular Retroversion

Abstract: We prospectively collected clinical data during the period 2001–2006 on 60 hips with symptomatic femoroacetabular impingement that had radiographic evidence of acetabular retroversion defined as a crossover sign on an adequate anteroposterior radiograph or retroversion on magnetic resonance imaging or computed tomography. Our treatment algorithm for acetabular retroversion used measurements of acetabular coverage (lateral center edge angle and the posterior wall sign) and condition of acetabular cartilage to d… Show more

“…There is only one study comparing the surgical treatments for acetabular retroversion. In this study, Peters et al [32] differentiated between the two surgical treatments analyzing two different morphologic types of acetabular retroversion: one with a deficient posterior wall and one with sufficient posterior coverage. The followup after 4 years was comparable regarding the Harris hip score.…”

“…Our results are not valid for hips that present with a crossover sign only. Based on the data from others [32], acetabular rim trimming may be a valid option for treatment of hips with a positive crossover sign but concomitantly sufficient posterior coverage.…”

“…Hips with retroversion often experience damage in the posterior acetabulum, which has been postulated to result from one of two mechanisms [36]. The first mechanism to consider is decreased contact area and a resulting elevated contact stress in the posterior acetabulum [7,14,15]. The preferred treatment for this mechanism of damage is periacetabular osteotomy [14,37].…”

“…There is increasing concern that failure to recognize abnormalities of acetabular morphology leads to inadequate or inappropriate treatment of hip disease [7,[11][12][13][14]16]. Consequently, the question arises whether normal coverage of the femoral head can be defined on an AP pelvis radiograph.…”
