Amyloidosis of heart and liver: comparison of Tc-99m pyrophosphate and Tc-99m methylene diphosphonate for detection.

Abstract: A prospective, comparative study was made of the efficacy of technetium-99m pyrophosphate (Tc PYP) and technetium-99m methylene diphosphonate (Tc MDP) in detecting soft-tissue amyloidosis. Tc PYP and Tc MDP scans were obtained within ten-day intervals in seven patients with histologically proven amyloidosis. Tc PYP was a better scanning agent for soft-tissue amyloidosis in all patients. Cardiac and hepatic involvement were proved by autopsy in one patient. Involvement of the heart was confirmed by echocardiogr… Show more

“…'wTc-MDP normally binds to the surface of bone mineral in areas of active bone turnover [17]. wTc pyrophosphate is also used for bone scans, and there have been reports of its uptake at sites where AL amyloid has been deposited, in visceral organs, and joints [12,13,18]. The isotope vehicles are thought to bind to the calcium present in amyloid deposits.…”

“…However, many of these patients are often unable to undergo the invasive procedures necessary to obtain specimens, and a reliable and sensitive in vivo diagnostic technique is, therefore, required to make the diagnosis. Encouraged by reports in patients with AL amyloid, of the uptake of "Tc-labelled methylene diphosphonate ("Tc-MDP) and "Tc-Iabelled pyrophosphate [11][12][13], we investigated the use of bonescans in the diagnosis of joint disease due to pr microglobulin-associated amyloid.…”

“…All seven patients had abnormal cardiac and/or hepatic 99m Tc-PYP uptake, compared to only four patients who were injected with 99m Tc-MDP. 99m Tc-PYP was found to be a better scanning agent for softtissue amyloidosis and for cardiac localization of amyloid [54]. A year later, Eriksson et al reported less convincing results for 99m Tc-PYP.…”

“…[2][3][4][5] Pulmonary amyloidosis is classified into tracheobronchial and parenchymal types according to the site of the amyloid deposition. 6 We describe a patient with tracheobronchial amyloidosis, in whom SPECT demonstrated localized 99 mTc-HMDP deposition.…”
