Summary1. Ampicillin concentrations were determined in serum and bile after intravenous injection into patients with T-tube bile drainage of 1 gram ampicillin before and during probenecid medication. The concentrations were followed up to fifteen hours after injection. 2. Probenecid increased the half-life of ampicillin in serum from 74 minutes to 137 minutes. 3. Ampicillin concentrations in bile were higher following probenecid medication and a concentration over 5 /g/ml was obtained for 3 h longer than before probenecid. 4. The ampicillin concentrations in bile were approximately the same as those in serum both before and during probenecid medication suggesting passive transport of ampicillin from blood to bile. 5. A combined treatment of ampicillin and probenecid might be of clinical value in the therapy of cholangitis and typhoid carriers.

Os autores, utilizando a técnica da diluição seriada em meio líquido, deter minaram as concentrações mínimas inibitórias da ampicüina e da hetacilina em 150 amostras de Salmonella typhi, isoladas em sua totalidade a partir de hemoculturas, no Estado da Guanabara.Obteve-se como média das concentrações bacteriostáticas, 0,545 mcg/ml para ampicilina e 0,720 mcg/ml, no caso da hetacilina.Em um a das etapas do processo de fer mentação empregado para a obtenção da penicilina G, tem-se a presença de elevado teor de ácido 6-aminopenicilânico, ao qual podem ser ligados vários radicais ou gru pos prostéticos. Por tal mecanismo, foi possível a síntese de diversas substâncias, que receberam a denominação genérica de penicilinas semi-sintéticas e que revelaram uma efetiva ação bactericida sobre grande número de espécies bacterianas.Alguns destes novos antibióticos sinte tizados, em particular a ampicilina e a he tacilina, dem onstraram nos ensaios labo ratoriais um amplo espectro antibacteriano, inclusive agindo sobre determinados grupos de bactérias Gram negativas, re presentantes da família Enter obacteriaceae, que notoriamente discreta sensibili dade m anifestam à penicilina.Acresce à im portância destes dois novos farmacos, em vista dos resultados das ex perimentações farmacológicas, que assina laram como uma propriedade característica e comum, a excreção por períodos prolon gados através dos duetos biliares, em con centrações muito acima daquelas registra das na corrente sanguínea (1).Com as atenções focalizadas nesta pro priedade, evidenciou-se, nas investigações microbiológicas, a franca atividade "in vitro" da ampicilina e da hetacilina sobre alguns dos sorotipos do gênero Salmonella, notabilizando-se principalmente esta ação na espécie Salmonella typhi. Assim, ba seados nesta característica, em princípio estudada apenas na ampicilicina, foi dado ensejo ao surgimento de vários outros en saios, que se detiveram em analisar a sua eficiência terapêutica na difícil incumbên cia de tentar a erradicação do bacilo tí-fico, de localização vesicular nos portado res (2,3,6,8,14).Considerando, portanto, a importância fundam ental representada pelo estado de portador crônico na manutenção do ciclo
