Amiodarone prophylaxis for atrial fibrillation of high-risk patients after coronary bypass grafting: a prospective, double-blinded, placebo-controlled, randomized study

Abstract: The incidence of post-operative AF among high-risk patients was significantly reduced by a prophylactic amiodarone treatment resulting in a shorter time of intensive care unit and hospital stay. Our data supports the prophylactic use of amiodarone in peri-operative period in patients at high risk for AF after CABG.

“…Some of them reported a reduced occurrence of AF,3844 whereas others found no statistically significant differences 4548. These differences in outcomes might be due to inconsistencies in dosing and administration strategies as some used intravenous amiodarone,38,40,48 others applied oral amiodarone,39,41,43,46,47 and still others used a combination of intravenous and oral amiodarone 42,44,45. However, out comes of prophylactic amiodarone use differed even within administration groups 3848.…”

“…The development of AF is extremely common after cardiac surgery and is associated with longer intensive care unit and hospital stays, increased morbidity and mortality, and higher utilisation of healthcare resources [6][7][8][9][10][11]. For these reasons, it is of utmost importance to identify patients who are at risk for the development of postoperative AF (POAF), and to take due precautions in the preoperative period.…”

“…Various studies regarding the causes of POAF have been conducted, including advanced age, obesity, left atrial dimensions, cardiac dysfunction, chronic respiratory failure, renal dysfunction, fluid balance, preoperative medications, inflammation, and fibrosis, but no definitive consensus has yet been reached and various factors are involved with the occurrence [1][2][3][4][5] (Fig. 1).…”

“…Atrial fibrillation (AF) is the most common complication after cardiac surgery, with an incidence of 16%-85%. Postoperative AF (POAF) often occurs in the early period after surgery, [1][2][3] and has an influence on stroke and early postoperative cardiac events, as well as on the long-term prognosis. Thus, it is important to prevent POAF.…”
