Amelanotic melanoma in the vicinity of acquired melanocytic nevi and not arising from agminated melanocytic nevi: Masquerading as pyogenic granuloma

Abstract: Amelanotic melanoma (AMM) presenting as pyogenic granuloma and occurring in the vicinity of acquired melanocytic nevi is rare. Herein, we report such a manifestation in a 68-year-old male who presented with the painful red nodule and multiple pigmented patches involving the left great toe. Histopathological examination of skin biopsy taken from the nodule with an immunohistochemical study using HMB45 and S-100 confirmed the diagnosis of AMM. Biopsy from the pigmented patch near the nodule showed features of me… Show more

“…There are several types of melanoma and one of them is amelanotic melanoma with no pigmentation or with minimal residual pigmentation. The absence of pigment makes diagnosing very difficult and reduces patient's chance to recover [2,3]. Currently, standard treat-ment of malignant melanoma includes surgical resection, radiotherapy and systemic therapy such as immunotherapy, molecular therapy and chemotherapy with dacarbazine (DTIC) [4].…”

“…12 Several cases of initial misdiagnosis were reported by pathologists, which upon careful investigations and rechecking for the melanoma markers turned out to be amelanotic melanomas. 13 There were reports of amelanotic melanoma cases that were initially misdiagnosed as a basal cell carcinoma, 12 pyogenic granuloma, 14,15 poorly differentiated adenocarcinoma of the rectum, 16,17 Bowen’s disease, 18 presenting as contact dermatitis, 19 simulating juvenile xanthogranuloma, 20 and as otitis media. 21 Using the histological criteria alone, differentiation of amelanotic anorectal melanoma from other malignant tumors is difficult, and immunostains are of important help to make the diagnosis accurately.…”
