Ambulatory venous pressure: Correlation with skin condition and role in identifying surgically correctible disease

Abstract: Ambulatory venous pressure should be used to quantify venous insufficiency and remains the reference standard test of the venous calf muscle pump. The tourniquet test should not be used to select patients for surgery since it cannot distinguish deep from superficial venous incompetence. Venous reflux is best localised using Duplex ultrasound.

“…Superficial venous incompetence is almost universal and is the predominant cause of venous hypertension in approximately half of limbs with venous ulcers, particularly in younger patients with good mobility 9. Venous function in patients with mixed deep and superficial disease should be investigated by ambulatory venous pressure measurements 10. This involves cannulation of a foot vein and the use of tourniquets to occlude incompetent superficial veins to select those patients who may benefit from superficial venous surgery.…”

“…14 In a study of 360 limbs, the mean ambulatory venous pressure for limbs with reflux in the superficial venous system only was 61 mmHg. 15 In this case, a rapidly disseminating lichen sclerosus manifested itself overlying a mildly varicosed long saphenous vein. It is proposed that venous pressure in this range acted as the stimulus for the Köebner response in the context of a rapidly generalizing lichen sclerosus.…”

“…In the longitudinal section the mean blood flow velocity in the vessels was determined. Both VFV and VFA were calculated by the mean blood flow velocity ( v m ) and the diameter ( d ϭ 2 r ) of the vessels with the formula VF ϭ v m ϫ r. 2 Division of VFV and VFA calculates the VAFI (Figures 1 and 2). VAFI measurement was performed at a constant ambient temperature.…”

“…Numerous data on venous hemodynamics can be obtained using plethysmographic methods or ambulatory venous pressure measurement, which is accepted as a standard test of venous insufficiency. [2][3][4][5][6] But the extent and restricted repeatability are limiting factors. Photoplethysmography, strain gauge, or air plethysmography only allow an approximate calculation of the competence of the venous system 7-9 because of widely dispersed standard and pathologic values.…”
