Alveolar adenoma of the lung: a clinicopathological description of a case of this very unusual tumour

Abstract: Alveolar adenomas are extremely rare, and are probably benign lung tumours of unknown histogenesis. This report describes a case of alveolar adenoma in a 43 year old white man, who presented with pleuritic chest pain. A chestx ray and computerised tomography scan demonstrated a solitary left lower lobe lung nodule. Although a positron emission tomography scan seemed to document the benign nature of the lesion, a thoracoscopic wedge resection was performed to alleviate the symptoms and verify the diagnosis. His… Show more

“…Additionally, epithelia are positive for type II pneumocyte markers, indicating that the epithelial layer is derived from type II pneumocytes (16). In cases where the epithelial layer is flattened in shape and resembles the endothelium, the absence of immunostaining for CD31 or CD34 vascular markers may help to distinguish alveolar adenoma from other pulmonary lesions (17,18).…”

“…Previous immunohistochemical studies have demonstrated that alveolar adenoma epithelia are positive for markers including pan-CK, epithelial membrane antigen and TTF-1 (16). Additionally, epithelia are positive for type II pneumocyte markers, indicating that the epithelial layer is derived from type II pneumocytes (16).…”

“…Alveolar adenoma (AA) is a rare pulmonary tumour of uncertain histogenesis. Only 25 cases of AA have been reported in the English medical literature (Table 1) (1–10). Yousem & Hochholzer initially described AA in 1986 (6).…”

“…AA is a solitary, well‐demarcated peripheral pulmonary lesion composed of a network of spaces lined with simple cuboidal epithelium that contain stroma ranging from thin inconspicuous strands of connective tissue to broad accumulations of spindle cells accompanied by a myxoid matrix (3). The epithelial component consists of type‐II pneumocytes (3, 5, 6, 8). Most authors agree that these tumours are histologically benign, and recurrence has never been reported after resection (3, 6, 9).…”
