The effects of middle ear pressure pulses were studied in 37 patients with a diagnosis of definite Meniere's disease and active vestibular symptoms, 31 of whom had failed to respond to medical treatment including diuretics prior to pressure treatment. The number of vertigo spells during the 6 months prior to treatment and for 6, 18 and 24 months after the start of treatment were evaluated. The functionality level scores were three or worse according to the American Academy of Otolaryngology--Head and Neck Surgery (AAO-HNS) criteria. The results of the study were as follows: 19 patients experienced freedom from vertigo spells; 15 patients reported a significant decrease in the frequency of vertigo spells; and 3 patients did not respond to pressure treatment and were subjected to gentamicin injections, I of whom consequently became deaf in the affected ear. No patient became worse in connection with pressure treatment. With the exception of the three patients who did not respond to pressure treatment, all patients reported an improvement in functionality of at least two levels according to the AAO-HNS functionality scale. No side effects or adverse events were observed during the 2 years of pressure treatment. Overall, the results indicate that pressure treatment provides efficient control of symptoms in patients with intractable forms of Ménière's disease without producing any adverse effects on the inner ear.

Treatment of idiopathic sudden hearing loss (SHL) is still a great problem for the physicians, due to the still unexplained etiopathogenesis of the disease. This retrospective study includes 115 patients with idiopathic SHL and compares two therapy regimens. Of the total number of patients, 51 had been primarily treated by hyperbaric oxygen (HBO) at 2.8 bar. The remaining 64 patients had been treated by pentoxifylline infusions in increasing and decreasing doses. Hearing thresholds were ascertained at the frequencies of 250, 500, 1,000, 2,000 and 4,000 Hz using pure-tone audiometry. The results after treatment showed a statistically significant improvement of hearing by 46.35 (SD ±18.58) dB hearing level (HL) in the group of patients treated with HBO, compared with an improvement of hearing by 21.48 dB HL (SD ±13.5) in the group of patients treated with pentoxifylline infusions (p < 0.001). Furthermore, hearing recovered to physiological values in 24 of 51 (47.1%) HBO-treated patients, compared to only 4 of 64 (6.2%) of the pentoxifylline-treated patients. Furthermore, after pentoxifylline treatment only 8/64 (12.5%) patients showed partial recovery, whereas after HBO therapy 21/51 (41.2%) patients improved up to a mild persistent hearing loss. This indicates strong evidence that HBO therapy is more effective in the treatment of idiopathic SHL than conventional rheological treatment. However, more prospective clinical trials should confirm the efficiency of HBO therapy and lead to its wider application.

Debate surrounding the need for evidence-based research in all fields of otolaryngology has recently emerged. A review of English language articles dealing with the treatment of Menière's disease published from 1989 to 1999 was performed. The papers were classified according to article type (observational studies, non-clinical studies, controlled trials, randomised controlled trials) and according to the grade of evidence that was offered. A total of 128 publications were reviewed, of which only three were randomised controlled trials and a further three were well-designed controlled studies without randomization. The majority of papers were non-experimental clinical descriptive studies or case series. Data from this review would suggest that more comprehensive evidence-based studies are required in order to settle areas of confusion surrounding treatment of this relapsing condition
