Background: Integrated-pathy aims to integrate modern medicine with traditional systems via applying the holistic approach of Ayurveda, Yoga, Yagya, Panchkarma and natural medicine. This is important for addressing the challenges surrounding the delivery of long-term palliative care for chronic ailments including cancer. The prime intent of this study was to substantiate the underlying hypothesis behind the differential and integrative approach having a positive impact on Quality of Life of cancer patients. Study Design: Observational study Methods: A standardized questionnaire was developed and used, after obtaining written informed consent from patients to assess the impact of Integrated-pathy on patients (n=103) diagnosed with cancer receiving care at Patanjali Yoggram. The research was carried out over an eight-month period (December-2021 to July-2022). For the significance of the pre- and post-treatment QoL ratings, Wilcoxon signed-rank pair test, Kolmogorov-Smirnov test and other descriptive statistics techniques were explored. Results: A total of 103 patients seeking cancer special-healthcare were interviewed, out of which 39 (37.86%) were selected based on the inclusion/exclusion criteria with age (25-65 years), period of Integrated-pathy received (≥1 months), allopathic treatment received prior to opting for Integrated-pathy. Results revealed a significant increase in the QoL (17.91%) after receiving the integrated therapy over a course of at least one month. Further, a significant reduction in cancer related pain followed by an increase in QoL index was reported in the patients. Wilcoxon rank test revealed significant pairing (p<0.0001) with validation of the model using Kolmogorov-Smirnov test. Conclusions: In order to enhance evidence-based support to the Integrated-pathy, well-defined clinical indicators that can be assessed and monitored over time are required. Clinical researchers should add Integrated-pathy into their proposed interventions and undertake similar studies to provide long-term benefits.

Persistent Mycobacterium tuberculosis (Mtb) attributable to resistant strains has increased the burden leading to poor quality of life to the patients. Drugs of herbal origin have been identified as potential sources of new anti-tubercular agents for the treatment of this disease and emerging resistant strains of the bacterium. In this research, therefore, five herbs/plants that have been indicated in the treatment of Mycobacterium tuberculosis in the traditional/folklore medicine were evaluated for their potential use in the treatment of Mycobacterium tuberculosis infection. Garcinia kola, Picralina nitida, Mitracarpus villosus, Vernonia amygdalina and Securidaca longipedunculata were assessed for their anti-tubercular activity using standard methods. G. kola and V. amygdalina having better activity were assessed for their synergistic activity. Doses displaying in-vivo activity were established. The activity of the synergistic mix with Rifampicin was evaluated in a resistant Mtb strain. Two plants; G. kola (MIC of 125 mg/ml) and V. amygdalina (MIC of 250 mg/ml) performed better than others. The synergistic ratios of V. amygdalina, G. kola of 1:1, 2:1, 4:1 and 8:1 produced enhanced activity. An 8:1 ratio mix dose of 2500 mg/kg and above exhibited anti-tubercular activity ex-vivo. The plasma concentration of the hybrid mix interacted with Rifampicin to exhibit antitubercular activity in a resistant strain of the bacterium. V. amygdalina and G. kola in suitable ratio exerts synergistic anti-tubercular activity and interacts with Rifampicin to inhibit resistant tubercular bacilli growth.INTRODUCTION: Tuberculosis (TB) is one of the main causes of mortality and morbidity globally.

Guggulsterone phytosomes: A novel approach to alleviate hyperlipidemia in high-fat diet-fed rats

Introduction: Guggulsterone, derived from Commiphora mukul, is a potent hypolipidemic medication with low bioavailability and water insolubility. To address these challenges, the study aimed to formulate phytosomes and evaluate the efficacy of guggulsterone phytosomes (GPs) in reducing hyperlipidemia in rats on a high-fat diet. Methods: GPs were formulated by incorporating soya lecithin with a suitable solvent to enhance their efficacy against hyperlipidemia induced by a high-fat diet in rats. The optimized GPs were characterized, and in vitro drug release pattern was examined. The hypolipidemic effect of GPs (25 mg/kg body weight) was evaluated in Sprague Dawley rats over 28 days. Results: The GPs demonstrated favorable entrapment effectiveness with a particle size of 145.4 nm and a zeta potential of -17.8 mV. In terms of drug release, the GPs exhibited better stability and bioavailability, with a release of 92.07 ± 1.67% within 24 hours, compared to pure guggulsterone, which only released 28.07 ± 0.81%. GPs elevated the levels of high-density lipoprotein (HDL) and significantly (P<0.05) reduced triglycerides (TG), low-density lipoprotein (LDL) levels, and total cholesterol (TC), compared to their respective control groups. Moreover, GPs showed substantial (P<0.05) antioxidant activity, reduced steatosis, inflammatory cell, and fat cell infiltration in the liver tissue. Conclusion: GPs exhibited hypolipidemic activity in rats with high-fat diet-induced hyperlipidemia compared to pure guggulsterone. These findings emphasize the potential of GPs as an effective therapeutic intervention for managing hyperlipidemia, surpassing the conventional use of the pure compound.
