Thirty preeclamptic, 30 normotensive pregnant and 25 healthy nonpregnant women were analyzed with regard to the antioxidant system (thiols and superoxide dismutase, SOD). In preeclampsia both plasma and lysate thiol levels were significantly lower compared to controls (p < 0.001). SOD levels were higher in normotensive pregnant women, but were lower in preeclamptic compared to nonpregnant women. This finding of antioxidant changes in the red blood cell suggests that red cell dysfunction is fundamental in the development of preeclampsia, and similar alterations in the balance of the thiol could be present across the endothelial cell membrane.

The effect of the thiol containing compounds N-acetylcysteine and captopril on glutathione metabolism in human erythrocytes has been investigated non-invasively using 'H spin echo NMR. N-Acetylcysteine was found to increase the reduced form of glutathione while captopril increased the oxidized form, but no changes in the total glutathione concentration were observed. Incubation of the cells with buthionine sulphoximine to inhibit de novo glutathione synthesis did not affect the response. The results show that these compounds act by altering the redox balance of glutathione rather than by stimulating its synthesis, and that their mechanisms of action are different, and not simply due to the presence of the thiol group.

The sulphydryl population on erythrocyte membrane is shown to vary as a function of the nutritional status of the cell. When an assay based on reaction with Eliman's reagent and controlled conditions were used the mean values (SEM) for the sulphydryl population on the membranes of normal erythrocytes incubated overnight in the presence and absence of glucose were found to be 3-29 (0.27) and 2 56 (0.25) million sulphydryl functions per cell respectively. Under identical conditions rheumatoid erythrocytes incubated in the presence and absence of glucose were found to have a significantly lower sulphydryl population-1-54 (0.08) and 1-15 (0.08) million respectively.The predominant concentrations of sulphydryl groups on the membrane are found at sites on the transmembrane proteins and, in particular, on the hexose transport protein. By influencing the nutritional status of the cells significant differences in activity between the normal and diseased state have been identified and these may have a role in the aetiology of rheumatoid arthritis by altering the response of cells to oxidative stress.
