Alteration in the Recommended Dosing Schedule for Risperidone

Abstract: These findings suggest that the recommended dose schedule should be altered to one that recommends a less rapid titration (over 6 days to a week) and that the dose increments consist of 0.5-2 mg/day.

“…All these data seem to suggest that it may be apparently difficult to establish RSP optimal individual oral dosage and dose titration in some cases [19]. In any case it seems interesting that the suggested minimal effective dose of RSP based on PET-measured D2 and 5HT2A receptor occupancy is 4 mg/day [27].…”

“…Randomized studies comparing olanzapine and risperidone in schizophrenia treatment have had mixed results regarding their relative efficacy [14–16]. Risperidone's safety profile has resulted in recommendations that risperidone doses be kept below 6 mg per day [17–19], whereas olanzapine has been associated with more weight gain than risperidone [20]. The one randomized comparison of olanzapine ( n = 172) and risperidone ( n = 167) that evaluated maintenance of response found an advantage for olanzapine (88% vs. 68% maintained response until week 28, Kaplan–Meyer P = .001) [14].…”
