alpha-BLOCKERS FOR THE TREATMENT OF CHRONIC PROSTATITIS IN COMBINATION WITH ANTIBIOTICS

Abstract: The use of alpha-blockers is justified, not only for prostatodynia, but also for abacterial and bacterial prostatitis. In the latter case alpha-blockade not only caused enhanced clinical improvement but also reduced the recurrences as defined by expressed prostatic secretion positive segmental cultures.

“…34 Recently it has been reported that the use of alphablockers in combination with antibiotics was more ef®ca-cious than antibiotics alone. 40 Direct injection of antimicrobial agents into the prostatic tissue has been suggested in the past, but has not achieved widespread approval, 41 while recently the anal submucosal route of antibiotics administration has been proposed for the treatment of chronic bacterial prostatitis, but the results of this report have not been con®rmed by others yet. 42 In cases of CBP with prominent prostatic calci®cation, surgery has been suggested, especially if there are a large number of prostatic calculi.…”

“…Barbalias et al, 40 based on their video-urodynamic ®nd-ings during micturition (distal narrowing of the prostatic and membranous urethral segment) in patients suffering from`painful male urethral syndrome', treated this group of patients with alpha-blockers. The majority of them responded favorably to treatment, but maintained the good results only with a long-term treatment schedule (up to eight months).…”

“…Besides ef®cacy, safety, blood-pressure effects, therapeutic dose range, and cost, other considerations of alpha-blockers should include their effects on plasma lipids, 80 apoptosis, 81,82 erectile function, 83,84 and prostatitis. 85 In the future, the development of a true subtypeselective alpha-1 antagonist may lead to signi®cant advances in the medical treatment of BPH. It remains to be seen whether a subtype-selective agent will be able to reduce side effects while maintaining ef®cacy.…”

“…Combination therapy [30] included a daily dose of the alpha-adrenoceptor blocker alfuzosin (10 mg, extended-release formulation) [33] and a S. repens extract (640 mg day −1 ). This therapeutic regimen was administered for 6 weeks to all patients.…”
