Levocetirizine hydrochloride (LVC) is an antihistaminic drug that is repurposed for the treatment of alopecia. This investigation is targeted for formulating LVC into cationic ceramide/phospholipid composite (CCPCs) for the management of alopecia. CCPCs were fabricated by ethanol-injection approach, through a central composite experiment. CCPCs were evaluated by inspecting their entrapment efficiency (EE%), polydispersity index (PDI), particle size (PS), and zeta potential (ZP). The optimum CCPCs were additionally studied by in-vitro , ex-vivo , in-silico , and in-vivo studies. The fabricated CCPCs had acceptable EE%, PS, PDI, and ZP values. The statistical optimization elected optimum CCPCs composed of 5 mg hyaluronic acid, 10 mg ceramide III, and 5 mg dimethyldidodecylammonium bromide employing phytantriol as a permeation enhancer. The optimum CCPCs had EE%, PS, PDI, and ZP of 88.36 ± 0.34%, 479.00 ± 50.34 nm, 0.377 ± 0.0035, and 20.20 ± 1.13 mV, respectively. The optimum CCPC maintained its stability for up to 90 days. It also viewed vesicles of tube shape via transmission electron microscope. The in-silico assessment resulted in better interaction and stability between LVC and vesicle components in water. The ex-vivo and in-vivo assessments showed satisfactory skin retention of LVC from optimum CCPCs. The histopathological assessment verified the safety of optimum CCPCs to be topically applied. Overall, the optimum CCPCs could be utilized as a potential system for the topical management of alopecia, with a prolonged period of activity, coupled with reduced LVC shortcomings.

The medical term for baldness is alopecia. It refers to the condition where hair is lost from one or more areas of the body, commonly from the scalp. Balding can result from various factors, including genetic predisposition, environmental triggers, exposure to chemicals, medications, nutritional deficiencies, excessive stress, or prolonged illness. Depending on the pattern of hair loss and its underlying cause, alopecia is categorized into several types. Among these, androgenetic alopecia and alopecia areata are of primary concern. Presently, there are numerous synthetic treatments available for managing alopecia (both areata and androgenetic alopecia), including minoxidil, corticosteroids, zinc, dithranol, systemic cortisone, tretinoin, irritants, immunosuppressants, azelaic acid, and finasteride. However, these treatments often fail to provide satisfactory and long-lasting results for individuals with alopecia. Moreover, these synthetic products are associated with adverse effects, such as itching, redness, dermatitis, flaking, and irritation. Natural remedies have been explored to address the challenge of hair loss. A variety of herbs have demonstrated efficacy in managing alopecia. These natural remedies operate through mechanisms such as nutritional support, 5-α-reductase and DHT (dihydrotestosterone) inhibition, aromatherapy, and improved scalp blood circulation. Utilizing these natural treatments offers several advantages, including patient adherence, minimal side effects, easy accessibility, affordability, and diverse modes of therapeutic action for hair loss management. In this review, we highlighted the causes of alopecia and its promising, cost-effective treatments, which might significantly alter the lives of those impacted by baldness by providing them with a road to greater self-assurance, well-being, and quality of life.

Background Living organisms are continuously exposed to multiple internal and external stimuli which may influence their emotional, psychological, and physical behaviors. Stress can modify brain structures, reduces functional memory and results in many diseases such as skin disorders like acne, psoriasis, telogen effluvium, and alopecia areata. In this review, we aim to discuss the effect of secretome on treating alopecia, especially alopecia areata. We will shed the light on the mechanism of action of the secretome in the recovery of hair loss and this by reviewing all reported in vitro and in vivo literature. Main body Hair loss has been widely known to be enhanced by stressful events. Alopecia areata is one of the skin disorders which can be highly induced by neurogenic stress especially if the patient has a predisposed genetic background. This condition is an autoimmune disease where stress in this case activates the immune response to attack the body itself leading to hair cycle destruction. The currently available treatments include medicines, laser therapy, phototherapy, and alternative medicine therapies with little or no satisfactory results. Regenerative medicine is a new era in medicine showing promising results in treating many medical conditions including Alopecia. The therapeutic effects of stem cells are due to their paracrine and trophic effects which are due to their secretions (secretome). Conclusion Stem cells should be more used as an alternative to conventional therapies due to their positive outcomes. More clinical trials on humans should be done to maximize the dose needed and type of stem cells that must be used to treat alopecia areata.
