Alopecia Areata: a Comprehensive Review of Pathogenesis and Management

Abstract: Alopecia areata is a common hair loss condition that is characterized by acute onset of non-scarring hair loss in usually sharply defined areas ranging from small patches to extensive or less frequently diffuse involvement. Depending on its acuity and extent, hair loss is an important cause of anxiety and disability. The current understanding is that the condition represents an organ-specific autoimmune disease of the hair follicle with a genetic background. Genome-wide association studies provide evidence for… Show more

“…The characteristic patch of AA is usually round or oval, bald and smooth. Occasionally, AA may progress to involve all scalp hairs (alopecia totalis [AT]) or other body hairs beyond the scalp (alopecia universalis [AU]) . AT and AU have long been viewed as highly challenging conditions, with no treatment modalities guaranteeing a satisfactory response.…”

“…In alopecia areata, T helper infiltrates around hair papilla and T cytotoxic in it, and both types are present in the bulge (Hong et al, ). The pathogenesis of alopecia areata is closely related to IFNγ‐induced chemokines and cytotoxic CD8 T cells, but in our patient it was connected with III type of allergic reaction (an immune complex‐mediated hypersensitivity reaction) (Trüeb & Dias, ). It is probably related to Menier's disease incidence, where the immune response to autoantigens developed (Kim et al, ).…”

“…The changes are manifested by nail pitting or making it lustreless and rough. The lesions may take various forms: trachyonychia, punctate leukonychia, spotted or red lunulae, transverse nail grooving (Beau's lines) or the rare separation of the nail plate from the matrix with nail loss (onychomadesis) [7].…”
