Alloimmunisation during pregnancy in Greece: need for nationwide HDFN prevention programme

Abstract: Our results depict differences in the delivery of health care between migrants and Greek women, as well as the heterogeneity in practices for the prevention of haemolytic disease of foetus and newborn in Greece and highlight the need for the implementation of nationwide guidelines.

“…The prevalence of maternal clinical significant antibodies in our institution was 0·7%, with the Rh system being the most common antibodies. These results are consistent with other studies (Foudoulaki-Paparizos et al, 2013;Smith et al, 2013;Pal & Williams, 2015). Of six cases that were diagnosed with fetal anaemia by US Doppler, five were because of Rh antibodies and one because of anti-K. Current UK guidelines recommend that regular monitoring is performed only for anti-D, anti-K and anti-c antibodies (White et al, 2016).…”

“…While the widespread introduction of Rhesus (Rh) immunoglobulin represents one of the great success stories in modern obstetrics, clinicians continue to encounter cases of RhD alloimmunization. In contemporary obstetrics, women with RhD antibodies detected undergo serial antibody level measurements antenatally; those whose levels exceed a certain critical threshold are referred for ultrasound surveillance.…”

“…Lastly, despite the fact that the role of prior pregnancies in the RBC alloimmunisation of female recipients can not be formally ruled out, it does not explain the high ratio of observed JK1 alloimunisation. In pregnant women, RBC antibodies are predominantly directed against RH and KEL antigens (Foudoulaki-Paparizos et al, 2013;Pal & Williams, 2015).…”
