Background:Humeral head fractures and their postoperative outcome remain a challenging problem in surgical daily routine. Predictive factors for loss of fixation are rare.Objective:Determination of predictive factors for the failure of osteosynthesis with the loss of fixation or migration of screws in humeral head fractures.Method:From 1995 to 2011, 408 patients with proximal humeral fractures [mean age 66.6 years, 50.9-82.3 years] and osteosynthesis were analyzed. Two hundred and three received open reduction internal fixation (ORIF) with the PHILOS® plate. The non-locking plate was used in 80, the locking plate in 16 and humeral head prosthesis in 26 patients, in addition to 23 patients undergoing other procedures. Intraoperative reduction that achieved an anatomical alignment of the medial aspect of the humerus (humeral calcar) was assessed in 94 patients by postoperative X-ray analysis. The loss of fixation was evaluated by a follow-up of three to five X-rays and measurement of the humeral tip-apex-distance (HTAD).Results:For stable fixed fractures with an intact calcar, percentual HTAD was significantly higher than for unstable fixed fractures (p=0.04). Morbidity, such as hypertension, orthopedic operations or diabetes, strongly influenced the HTAD, while postoperative passive motion treatment modestly affected the HTAD over time.Conclusion:The anatomic reconstruction of the calcar, leading to stable fixation of humeral head fractures, can significantly prevent an overproportioned decrease in the HTAD in postoperative X-rays and seems to be vital in multimorbid patients. Measurement of the HTAD over time delivers a tool for early detection of secondary loss of fixation.

Purpose Varus failure is one of the most common failure modes following surgical treatment of proximal humeral fractures. Straight antegrade nails (SAN) theoretically provide increased stability by anchoring to the densest zone of the proximal humerus (subchondral zone) with the end of the nail. The aim of this study was to biomechanically investigate the characteristics of this Bproximal anchoring point^(PAP). We hypothesized that the PAP would improve stability compared to the same construct without the PAP. Methods Straight antegrade humeral nailing was performed in 20 matched pairs of human cadaveric humeri for a simulated unstable two-part fracture. Results Biomechanical testing, with stepwise increasing cyclic axial loading (50-N increments each 100 cycles) at an angle of 20°abduction revealed significantly higher median loads to failure for SAN constructs with the PAP (median, 450 N; range, 200-1.000 N) compared to those without the PAP (median, 325 N; range, 100-500 N; p = 0.009). SAN constructs with press-fit proximal extensions (endcaps) showed similar median loads to failure (median, 400 N; range, 200-650 N), when compared to the undersized, commercially available SAN endcaps (median, 450 N; range, 200-600 N; p = 0.240). Conclusions The PAP provided significantly increased stability in SAN constructs compared to the same setup without this additional proximal anchoring point. Varus-displacing forces to the humeral head were superiorly reduced in this setting. This study provides biomechanical evidence for the Bproximal anchoring point's^rationale. Straight antegrade humeral nailing may be beneficial for patients undergoing surgical treatment for unstable proximal humeral fractures to decrease secondary varus displacement and thus potentially reduce revision rates.

Mit einer Häufigkeit von 2-3 % aller Schulterluxationen ist die traumatische hintere Luxation eine selten vorkommende Verletzung des Schultergürtels, welche initial leicht übersehen werden kann [1]. Eine zeitlich verzögerte Diagnosestellung kann einen nicht unbedeutenden Einfluss auf eine evtl. notwendige chirurgische Intervention mit sich ziehen. Lässt sich eine traumatische hintere Schulterluxation nativradiologisch und klinisch nicht eindeutig ausschließen, sollte eine zeitnahe bilaterale Computertomographie (CT) der Schulterregion durchgeführt werden, um das Ausmaß der Verletzung zu detektieren und um eine rasche weiterführende Behandlung einzuleiten [2]. Da es im Rahmen einer traumatischen hinteren Schulterluxation nahezu immer (86 %) zu der Entstehung einer inversen Hill-Sachs-Läsion (anteromediale Humeruskopfimpressionsfraktur) kommt, ist diese mit ihrer variierenden Defektgröße und unterschiedlich vorkommenden Lage mitunter richtungsweisend für die weitere Behandlung [3,4].Als eine absolute Operationsindikation sollte das Auftreten eines Engagements gesehen werden. Das Engagement wird durch die Lage und Ausdehnung der inversen Hill-Sachs-Läsion in Bezug auf den posterioren Glenoidbereich in Abhängigkeit einer funktionellen Schulterposition (Adduktion und Innenrotation) definiert [5][6][7]. Zur operativen Behandlung der inversen Hill-Sachs-Läsion wurden sowohl anatomische wie auch nichtanatomische und prothetische Verfahren beschrieben [8][9][10][11][12].Nicht jedes dieser operativen Verfahren ist zu jedem Zeitpunkt der Verletzung anwendbar. Auch spielen die Defektgröße und -lage der inversen HillSachs-Läsion eine wesentliche Rolle. So kann in der Frühphase (erste 2 Wochen nach dem Trauma) bei einer Humeruskopfimpressionsfraktur <40 % der anteromedialen Gelenkfläche meist eine erfolgreiche Imprimathebung mit anschließender autologer oder allogener Spanunterfütterung und zusätzlicher osteosynthetischer Fixierung durchgeführt werden. Bei einer sehr großen inversen HillSachs-Läsion (>45 % der Gelenkfläche), einem länger verbleibenden Luxationszustand bzw. bei einer chronisch verhakten hinteren Schulterluxation (>4 Wochen) lässt sich diese operative Technik in der Regel nicht mehr erfolgreich umsetzen. In diesen Fällen erweist sich die Segmentrekonstruktion mit einem Allograft als eine gute Alternative [4,13]. AllograftsAllografts (allogene Knochentransplantate) werden zur Behandlung von diversen traumatologischen und orthopädi-schen Verletzungen/Erkrankungen mit größerem Knorpel-/Knochensubstanzverlust eingesetzt [14,15]. Deren Anwendung nahm über die letzten Jahre kontinuierlich zu [16] BehandlungsalgorithmusBei Vorliegen einer großen (γ-Winkel >90°oder α -Winkel >50°) bis sehr großen (γ-Winkel >115°oder α -Winkel >70°) inversen Hill-Sachs-Läsion mit einhergehendem Engagement bei Adduktion und Innenrotation des Oberarmkopfes sowie bei verhakten und chronisch verhakten Formen (>4 Wochen) wurde die Operationsindikation gestellt.
