Allodynia Is Associated With Initial and Sustained Response to Acute Migraine Treatment: Results from the American Migraine Prevalence and Prevention Study

Abstract: The use of triptan medication was associated with significantly better 2hPF (except vs ergot alkaloids) and significantly better 24hPR outcomes compared with other acute medication categories. The presence of allodynia significantly increased the likelihood of an inadequate treatment response for both of these outcomes. Triptan use was generally associated with the best outcomes. Because allodynia was associated with inadequate outcomes for all medication groups, we suggest that allodynia is an area of unmet t… Show more

“…Treatment of migraine, and particularly chronic migraine, remains challenging, and numerous therapeutic drugs are non‐specific, sometimes inefficient, with side effects and contraindications (Serrano et al ., ; Schytz et al ., ). Cutaneous allodynia in chronic migraine is associated with inadequate outcomes for any anti‐migraine medication (Lipton et al ., ). Furthermore, a significant proportion of chronic migraine cases are due to anti‐migraine medication abuse or misuse (Bigal and Lipton, ; Lipton et al ., ), highlighting the need for new medications, including new prophylactic approaches to prevent the chronification process.…”

“…Cutaneous allodynia has been associated with an increased risk of attack recurrence and migraine chronicity . Cutaneous allodynia also predicts inadequate 2‐hour pain‐free and 24‐hour pain relief outcomes . Diagnostic vigilance remains important, and all consulting patients, especially women, should be queried about the presence of allodynia.…”

“…21,41,42,47 Cutaneous allodynia also predicts inadequate 2-hour pain-free and 24-hour pain relief outcomes. 46,[48][49][50] Diagnostic vigilance remains important, and all consulting patients, especially women, should be queried about the presence of allodynia. Unilateral pain and headaches that are pounding or pulsatile are statistically more common in women than men, but the differences are small and may not be clinically relevant.…”

“…In reference to allodynia, which has been considered a pathophysiological correlate of clinical progression, we observed a statistically significant and greater reduction in those patients with a decrease in headache frequency of ≥50%. We also analyzed other migraine features, observing higher proportions of aura, both visual and sensory, after 10 years.…”
