The lymphocyte transformation test (LTT) was used to elucidate whether certain side effects induced by radiographic contrast media have an immunologic etiology. Groups studied were: 8 patients who had previously experienced adverse reactions in association with urography, 6 patients who underwent urography without notable side reactions, 17 occupationally exposed nurses, and 9 unexposed controls. The lymphocytes from 2 hypersensitive patients and from II nurses exhibited a positive proliferative response to amidotrizoate. Five nurses who had shown a positive response, had a previous history of hypersensitivity reactions when handling contrast media, whereas the remaining 6 were free of symptoms. Amidotrizoatespecific memory cells were absent in patients who underwent urography without signs of hypersensitivity and in 7/9 of unexposed control subjects. Lymphocytes from patients sensitive to amidotrizoate cross-reacted to structurally related ionic contrast media while nonionic contrast agents did not induce proliferation of the lymphocytes. Thus, ionic radiographic contrast agents have antigenic properties in man. Irradiated mixtures of radiographic contrast media and serum proteins were, in general, not effective in inducing an LTT response.

AIM:The aim of the study was to evaluate the clinical relevance, sensitivity and specificity of in vitro blood test, Memory Lymphocyte ImmunoStimulation Assay (MELISA ® ), in genetically predisposed patients that suffer by autoimmune/inflammatory syndrome induced by adjuvants, after HPV-vaccination and that could have a high metal hypersensitivity. MATERIALS AND METHODS:Sixteen girls (aged 12-24 years) that developed long-lasting and invalidating somatoform symptoms occurring within 20 days postvaccination are included in this descriptive study. The hypersensitivity to five metals (aluminum, nickel, mercury, methyl mercury, and thimerosal) was measured by MELISA ® test. RESULTS:Seven girls showed negativity to all the five metals tested. The findings showed metal hypersensitivity only in nine patients: Toxicity to aluminum (two girls), reactivity to nickel (seven girls), followed by mercury (seven girls). CONCLUSION:The MELISA ® assay is neither sensitive nor specific in detecting metal hypersensitivity and associated chronic diseases, including autoimmune pathologies.

In this study, 18 patients with oral lichen planus (OLP), adjacent to amalgam fillings, were tested in vitro with an optimized lymphocyte proliferation test, MELISA (memory lymphocyte immunostimulation assay) and with a patch test. Twenty subjects with amalgam fillings but without oral discomfort and 12 amalgam-free subjects served as controls. The results show that patients with OLP have significantly higher lymphocyte reactivity to inorganic mercury, a corrosion product of amalgam, compared to control groups. Removal of amalgam fillings resulted in the disappearance of oral mucosal changes, thus indicating a causal relationship. Positive responses to phenylmercury (phenyl-Hg), a bactericidal agent in root fillings and in pharmaceutical preparations, were also noted in the oral lichen group but not in the control groups. Thus, low-grade chronic exposure to mercury may induce a state of systemic sensitization as verified by Hg-specific lymphocyte reactivity in vitro.
