Allergy Testing in Childhood: Using Allergen-Specific IgE Tests

Abstract: A variety of triggers can induce common pediatric allergic diseases which include asthma, allergic rhinitis, atopic dermatitis, food allergy, and anaphylaxis. Allergy testing serves to confirm an allergic trigger suspected on the basis of history. Tests for allergen-specific immunoglobulin E (IgE) are performed by in vitro assays or skin tests. The tests are excellent for identifying a sensitized state in which allergen-specific IgE is present, and may identify triggers to be eliminated and help guide immunoth… Show more

“…Despite these systems provided the result in the same unit, the result is not always equivalent. 3,[8][9][10] Euroline (Euroimmun, Medizinische Labordiagnostika, AG, Germany), an IgE determination system using immunoblotting assay has some advantages such as the requirement of only small amount of sera, no automate machine requirement, minimal hands-on time, appropriate for screening multiple allergen and cost-saving. While ImmunoCAP (Phadia, Uppsala, Sweden), a fluorescence/enzyme-immunoassay, has some advantages such as high through-put with a lot of number of sample load per day but it has some disadvantage such as automate machine requirement and expensive.…”

“…86 Food allergy Children can have positive immunoglobulin E test results indicating sensitization but not necessarily suffer from a clinical allergy. 87 For example, 17% of people are sensitized to a major food allergen, but only 2.5% have a clinical food allergy. 88 …”

“…More information about allergy testing can be found in a recent AAP clinical report on the subject. 18 Alternatively, the patient can be referred to an allergist-immunologist for allergy skin testing, interpretation of results, and education of the family about environmental triggers.…”

“…Serum allergenspecific immunoglobulin E (IgE) antibody tests may be performed, or the patient may be referred to a board-certified allergist for allergy skin testing to identify indoor allergens that are most likely to be clinically relevant for a patient who meets National Asthma Education and Prevention Program criteria for persistent asthma, which include patients taking a long-term controller medication as well as patients having symptoms >2 days per week or nocturnal symptoms more than twice per month. 17 Serum IgE testing and allergy skin testing are both appropriate methods of assessing allergic sensitivities; neither is clearly better than the other in identifying clinically important sensitizations, 18 and there is no lower age limit for either of these tests.…”
