IntroductionThe SARS-CoV-2 (COVID-19) pandemic was managed with sustained mass lockdowns to prevent spread of COVID-19 infection. Babies born during the early stages of the pandemic missed the opportunity of meeting a normal social circle of people outside the family home.MethodsWe compared 10 parentally reported developmental milestones at 12-month assessment in a cohort of 309 babies born at the onset of the pandemic (CORAL cohort) and 1629 babies from a historical birth cohort (BASELINE cohort recruited between 2008 and 2011).ResultsCompared with a historical cohort, babies born into lockdown appeared to have some deficits in social communication. Fewer infants in the pandemic cohort had one definite and meaningful word (76.6% vs 89.3%), could point (83.8% vs 92.8%) or wave bye-bye (87.7% vs 94.4%) at 12-month assessment. Adjusted log-binomial regression analyses demonstrated significant differences in social communication in the CORAL cohort compared with the BASELINE cohort: one definite and meaningful word (relative risk (RR): 0.86 (95% CI: 0.80 to 0.92)), pointing (RR: 0.91 (95% CI: 0.86 to 0.96)) and waving bye-bye (RR: 0.94 (95% CI: 0.90 to 0.99)).DiscussionParentally reported developmental outcomes in a birth cohort of babies born into lockdown during the COVID-19 pandemic may indicate some potential deficits in early life social communication. It must be noted that milestones are parentally reported and comparison is with a historical cohort with associated limitations. Further studies with standardised testing is required to validate these findings.ConclusionPandemic-associated social isolation may have impacted on the social communication skills in babies born during the pandemic compared with a historical cohort. Babies are resilient and inquisitive by nature, and it is hoped that with societal re-emergence and increase in social circles, their social communication skills will improve.

To the Editor,The recently introduced epithelial barrier hypothesis posits that the totality of environmental exposures, or the exposome, is responsible for the growing prevalence of an extensive list of disorders, including atopic conditions. 1 Environmental insults damage the epithelial barriers, including that of the skin, thereby setting off a cascade of detrimental events. The theory, coupled with the known genetic underpinnings of atopic dermatitis, asthma, food allergies, and other conditions, provides for a comprehensive picture of geneenvironment interactions that result in disease.The integrity of the skin barrier is important for maintaining health, and its impairment has been associated with a number of medical conditions. Early-life impairment of the skin barrier, as measured by transepidermal water loss (TEWL), has been associated with atopic dermatitis, 2 aeroallergen sensitization, 3 and food allergen sensitization. 4 Intriguingly, these associations are often independent of loss-of-function filaggrin mutations. Filaggrin plays an important role in epidermal integrity, and loss-of-function filaggrin mutations are a risk factor for the development of atopic dermatitis, food allergies, allergic rhinitis, and asthma. 5 However, the lack of overlap between infant TEWL values and subsequent atopic conditions compared with filaggrin mutations and atopic conditions is perplexing. We hypothesized that TEWL values in infancy may reflect environmental exposures and not genetic predisposition. The Imprint of Exposome on the Development of Atopic Dermatitis across the Lifespan: A Narrative Review

Atopic dermatitis (AD) is a chronic inflammatory skin condition that affects more than 200 million people worldwide, including up to 20% of children and 10% of the adult population. Although AD appears frequently in childhood and often continues into adulthood, about 1 in 4 adults develop the adult-onset disease. The prenatal period, early childhood, and adolescence are considered critical timepoints for the development of AD when the exposome results in long-lasting effects on the immune system. The exposome can be defined as the measure of all the exposures of an individual during their lifetime and how these exposures relate to well-being. While genetic factors could partially explain AD onset, multiple external environmental exposures (external exposome) in early life are implicated and are equally important for understanding AD manifestation. In this review, we describe the conceptual framework of the exposome and its relevance to AD from conception and across the lifespan. Through a spatiotemporal lens that focuses on the multi-level phenotyping of the environment, we highlight a framework that embraces the dynamic complex nature of exposome and recognizes the influence of additive and interactive environmental exposures. Moreover, we highlight the need to understand the developmental origins of AD from an age-related perspective when studying the effects of the exposome on AD, shifting the research paradigm away from the per se categorized exposome factors and beyond clinical contexts to explore the trajectory of age-related exposome risks and hence future preventive interventions.
