Background: Contact hypersensitivity to corticosteroids is increasingly reported and has been identified as a problem of considerable clinical relevance. The prevalence of positive patch tests to corticosteroids ranges from 0.2 up to 5%. Objective: The prevalence of positive patch tests to corticosteroids in Switzerland was determined in a multi-centre study of patients undergoing routine patch tests. Methods: As representatives of corticosteroid groups, the following substances were used for screening: tixocortol pivalate and hydrocortisone for group A (hydrocortisone type), hydrocortisone butyrate for group D (hydrocortisone butyrate type) and budesonide for both groups B (triamcinolone type) and D. Patients positive for at least one corticosteroid were retested with the screening series and 12 corticosteroids commonly used in Switzerland. Results: Among 3,016 consecutive patients, 65 individuals (2.2%) with a total of 106 positive reactions were found. Retesting showed a concordance of 70-98%, depending on the corticosteroid and the score of the positive reaction. In the subsequently tested corticosteroid series including 12 substances, 19 out of 56 screening-positive patients had a positive result to one or several corticosteroids. There were only few evident cross-reactive patterns in between the corticosteroids tested. Conclusions: Corticosteroids should be included in routine patch testing, because contact sensitization to a corticosteroid is of considerable practical importance. We confirm that as markers of corticosteroid sensitization tixocortol pivalate, budesonide and hydrocortisone butyrate may be suited, because there is no single corticosteroid which is a marker for all four corticosteroid groups. Patch test reactions of 2+ or higher have a better reproducibility than 1 + reactions.

Polyfunctional molecules in which different photochemically reactive chromophores are connected by rigid hydrocarbon framework are a subject of fascinating photochemistry.1-3) The intramolecular energy transfer (both singletsinglet and triplet-triplet) may occur from an 'antenna' group to other chromophore leading to chemistry different from that observed by direct excitation of that chromophore. [4][5][6][7] In the photochemistry of such multichromophoric molecules the evaluation of interaction between the chromophores, the mode and extent of local reaction at any chromophore after electronic excitation and possible role of energy transfer is of high mechanistic significance.Morrison established, through a series of elegant papers, [4][5][6][7] that intramolecular energy transfer (both singlet-singlet and triplet-triplet) occurred from the phenyl 'antenna' to C 17 keto group in the steroids by the way of through-bond mechanism. This lead to a different photochemistry observed by the direct excitation of ketone chromophore. Albini et al. 8,9) have demonstrated non-communicating reaction paths in some pregna-1,4-diene-3,20-dione. Since many steroidal drugs are commonly used and several reports on their phototoxic effects have been reported, [10][11][12][13] it was of interest to extensively study the aspect of competition between chemical reactions of the separated excited moieties incorporated in the rigid skeleton of the steroids. It was expected that such photochemical mechanisms might have some relevance for the mechanism of phototoxicity.Glucocorticosteroids are natural hormones with a steroidal structure derived from 5a-pregnane. These steroidal hormones with powerful anti-inflammatory effects are secreted by the cortex of adrenal gland. Semisynthetic derivatives of these hormones are widely used as drugs to treat inflammatory illness, including arthritis and asthma, and many of them are effective by topical use in dermatoses and other dermatological disease.Desonide (De,1) is a synthetic nonfluorinated corticosteroid for topical dermatological use. It is used to treat inflammation caused by a number of conditions such as allergic reactions, asthma and psoriasis.14,15) Desonide is very interesting from photochemical point of view because it bear two spatially separated chromophores i.e. cross conjugated dienone moiety in ring A and an isolated ketone at C 20 . Objective: The aim of this study was to investigate the frequency of contact sensitivity in patients with psoriasis, whether there was an association between clinical types and contact sensitivity, whether patch test is a factor that causes Koebner reaction and frequency of contact sensitivity against commonly used topical corticosteroids. Methods: Fifty patients with psoriasis and 50 healthy volunteers were included in this study and 'European standard series' and test units of active ingredients of some corticosteroids were performed on their upper back. The patches were read on hours 24, 48 and on day 7 in order to detect delayed allergic reactions and also Koebner reaction. The results of both groups were compared by using chi-square test. Results: At the end of the patch test allergic reaction was observed in 7 of 50 (14%) patients with psoriasis and 12 of 50 (24%) healthy volunteers. There was no statistically significant difference between allergic reaction of study group and healthy volunteers. There was no statistically significant difference between the clinical types of psoriasis and allergic contact sensitivity. The frequency of reaction increased in individuals having a positive sensitivity history to any substance in both patient and control groups. Reaction to topical steroids was not seen in any patients. Koebner phenomenon due to patch test was also not seen in any patients. Conclusion:We did not show any association between psoriasis and contact sensitivity in this study. We believe that contact allergens should be determined by using patch test in psoriatic patients with a positive history to any substance. Key words: Allergic contact dermatitis contact sensitivity, patch test, psoriasis Amaç: Bu çalışmada psoriasis hastalarındaki alerjik kontakt duyarlılık sıklığının saptanması, hastalığın klinik tipleri ile alerjik kontakt duyarlılık arasındaki ilişkinin belirlenmesi, yama (patch) testin psoriasisi tetikleyen bir faktör olup olmadığının gösterilmesi ve sık kullanılan topikal streoidlere karşı kontakt duyarlılık sıklığının tespit edilmesi amaçlandı.Yöntemler: Çalışmaya 50 psoriasisli hasta ve 50 sağlıklı gönüllü alınarak sırt bölgelerine 'Avrupa standart kontakt alerjen serisi' ve topikal steroid etken maddelerini içeren test üniteleri uygulandı. Yama testi alanları 48., 72. saatte ve hem Koebner reaksiyonu hem de geç reaksiyon oluşturma olasılığını da araştırmak amacıyla 7 gün sonra değerlendirildi. Her iki grup istatistiksel olarak ki-kare testi kullanılarak karşılaştırıldı. Bulgular:Yama testi sonucunda 50 psoriasisli hastanın 7'sinde (%14) kontrol grubunu oluşturan 50 sağlıklı gönüllünün 12'sinde (%24) alerjik reaksiyon izlendi. İki grup arasındaki fark istatistiksel olarak anlamlı değildi. Psoriasis klinik tipleri ile alerjik kontakt duyarlılık arasında istatistiksel olarak anlamlı bir ilişki saptanmadı.
