Allergic contact dermatitis caused by tetrahydrofurfuryl acrylate in the adhesive of clip‐on earrings

Background Several cases of allergic contact dermatitis (ACD) to the glucose sensor FreeStyle Libre have been reported. Isobornyl acrylate (IBOA) and N,N‐dimethylacrylamide (DMAA) are known culprit allergens. Objectives To evaluate patients with suspected ACD to FreeStyle Libre in a standardized manner, present causative allergens, and assess patient‐reported implications. Methods A total of 15 patients with suspected ACD to FreeStyle Libre were patch tested with the Swedish baseline series and a new medical device series. IBOA and DMAA were tested at 0.1% and 0.3% in petrolatum (pet.). Readings were performed on day (D) 3 and D7. Background data, details on skin reactions, and associated implications were assessed using a questionnaire. Results Thirteen patients were sensitized to IBOA and four to DMAA. Two positive reactions to IBOA and one to DMAA were seen only at 0.3% concentration on D7. Median duration of sensor use before dermatitis onset was 6 months. Half the number of the patients took precautions in everyday life due to sensor‐related skin reactions. Six patients discontinued sensor usage. Conclusions Patients with suspected ACD to glucose sensors should be evaluated with a relevant patch test series containing IBOA and DMAA. Adding the 0.3% pet. concentration is recommended. The reading on D7 is necessary.

BACKGROUND Cochineal red is an organic compound widely used in food, cosmetics, pharmaceuticals, textiles, and other fields due to its excellent safety profile. Poisoning caused by eating foods containing cochineal red is rare, and repeated atrial arrhythmia due to cochineal red poisoning is even rarer. CASE SUMMARY An 88-year-old Asian female patient was admitted to hospital due to a disturbance of consciousness. Twelve hours prior to presentation, the patient consumed 12 eggs containing cochineal red over a period of 2 h. At presentation, the patient was in a coma and had a score of 6 on the Glasgow Coma Scale (E2 + VT + M4). The patient’s skin and mucous membranes were pink. Electrocardiography (ECG) revealed rapid atrial fibrillation without any signs of ischemia. We prescribed cedilan and fluid replacement for arrhythmia correction. Shortly after admission, the atrial fibrillation corrected to a normal sinus rhythm. On the day 2 of admission, the patient had a sudden atrial flutter accompanied by hemodynamic instability and rapidly declining arterial oxygen saturation between 85% and 90%. The sinus rhythm returned to normal after two electrical cardioversions. Six days after admission, the skin color of the patient returned to normal, and the ECG results were normal. The patient was transferred out of the intensive care unit and eventually discharged after 12 d in hospital. At the 2-mo follow-up visit, the patient was in good health with no recurrence of arrhythmia. CONCLUSION Although cochineal red is a safe, natural food additive, excessive consumption or occupational exposure can induce cardiac arrhythmias.

Objective: Sensitive skin is a dermatological condition that is frequently observed and causes discomfort in individuals. Therefore, individuals with sensitive skin must carefully choose skin care and makeup habits. Our aims in this study were to conduct the first sensitive skin prevalence study in Turkey and to identify skin care and makeup habits. Methods: An online prospective controlled survey study was conducted among 1037 women. Participants with and without sensitive skin aged >18 years were included in the 3-month study. Results: Of the participating women, 82.73% were found to have sensitive skin. The most sensitive part of the face was the cheek (p < 0.001). Therefore, blush was the least frequently used by individuals with sensitive skin (p = 0.008) Conclusion: Sensitive skin is an important social and medical problem. The skin care and makeup habits of individuals with sensitive skin should be examined in detail and more help should be provided by dermatologists to these individuals.
