The immunogenicity of molecules shed by schistosomula into culture medium (antigens present in schistosomula-released products, SRP-A) has been studied. The results obtained show that SRP-A preferentially induce an IgE response when injected into rats, without the need for adjuvants. Moreover, anti-SRP-A IgE is cytotoxic in vitro for the larvae in the presence of macrophages, eosinophils or platelets, which have previously been demonstrated as being the three efficient killer cells for schistosomula in the presence of specific IgE. Immunofluorescence analysis locates the target antigens at the schistosomulum surface. Among the antigens recognized by anti-SRP-A IgE, two molecules of 26 and 22 kDa have been identified by sodium dodecyl sulfate polyacrylamide gel electrophoresis followed by western blotting.

The main patient series consisted of 415 Egyptians attending the Cairo Cancer Institute and comprising 286 bladder cancer, 97 breast cancer, 14 head and neck cancer and 18 gastrointestinal cancer cases. Also included in the study were 36 patients with active schistosomiasis and 89 health controls. Serum IgA, IgG, IgG subclasses, IgE, Schistosoma and Ascaris-specific IgE (RAST) and the acute-phase protein CRP were measured in all, or sub-sets, of the main patient group. The well-established increase in IgE and IgG levels, and the more recently reported increase in the levels of IgG3 and IgG4 subclasses in patients with schistosomiasis, were also found in bilharzial bladder cancer, indicating that humoral immunity persists in cancer-bearing patients. However, the plasma protein profile in bilharzial bladder cancer is further modified by significant increases in the levels of IgA, IgG, IgG1, IgG2 and CRP when compared to levels in patients with Bilharzia in the absence of neoplastic change. Patients with cancers not associated with parasitic infestation also had significant increases in their serum levels of IgG1, IgG2, IgG, IgE and CRP when compared to healthy Egyptian controls, but 41% of these non-bladder cancer patients showed IgE responses to previous parasitic infestations suggesting that any immunological response to cancer would be on the background of a variable non-specific increase of IgE.

Sera from Schistosoma mansoni-infected human patients were tested for total IgE levels and specific IgE and IgG antibodies to S. mansoni adult worm, cercaria, and egg antigen by ELISA. Of 50 sera, 28 were investigated by enzyme-linked crossed immunoelectrophoresis (ELCIE) to detect IgE reactivity to individual adult worm extract components. All sera showed increased total IgE levels. Specific IgE antibody levels to the different antigens varied; they were significantly correlated with each other but independent from total IgE. No correlation was found between specific IgG and any of the IgE antibody levels. Testing of the 28 individual sera by ELCIE revealed heterogeneous patterns. Seven sera were found to be nonreactive: three reacted with one precipitate, and the others reacted with between two and nine precipitates. However, in no case were identical patterns recognized, although four antigens reacted with about 80% of the sera. The number of bands detected by the individual sera depended neither on the levels of total IgE nor on those of specific IgE.
