Aldosterone breakthrough during angiotensin-converting enzyme inhibitor therapy

Abstract: The effectiveness of angiotensin-converting enzyme (ACE) inhibitors in the treatment of cardiac diseases after infarction and heart failure, and of renal disease such as diabetic nephropathy, has been reported from recent large-scale clinical trials on ACE inhibitors. However, effects of ACE inhibitors during long-term therapy have not always been optimal. In recent years, aldosterone breakthrough has been suggested as a factor potentially involved, in that aldosterone levels may not remain suppressed when an … Show more

“…Plasma aldosterone was not decreased. 19,20 Treatment did not affect 24-hour sodium excretion, clearance of sodium, or renal tubular sodium handling measured by lithium clearance (Table S2). Hematocrit and hemoglobin were decreased by about 5% in the candesartan treated group at month 12 21 (Table S2).…”

“…Aldosterone has detrimental effects on the cardiovascular system and synergizes with angiotensin II to increase blood pressure and produce cardiovascular remodeling and hypertrophy. Thus, combined treatment of aldosterone synthesis blockers and ACE inhibitors is reported to be more successful in treating cardiovascular disorders Sato & Saruta, 2003).…”

“…With angiotensin-converting enzyme inhibitors (ACE-I), aldosterone, which had decreased, is increased by the long-term administration (aldosterone breakthrough), 8) and the same cases are also observed with ARB. 9) Yoneda et al reported that this occurred in 23% of cases given candesartan.…”
