Aldosterone Antagonists in Heart Failure

Abstract: Aldosterone antagonists are an important pharmacologic therapy in the neurohormonal blockade necessary in the treatment of systolic heart failure. These drugs have been shown to decrease mortality and reduce hospital readmission rates. The major complication of aldosterone antagonists is hyperkalemia, which can be avoided with appropriate patient selection and diligent monitoring.

“…These medicines have been shown to reduce mortality and lessen hospital readmission speeds. The dominating complication of aldosterone antagonists is hyperkalemia, which can be averted to opportune patient choice and earnest monitoring [27]. By combining aldosterone with Mineralcoticosteroid Receptors MR, activating them as intracellular transcription elements.…”

“…Renal function and serum creatinine concentrations should be monitored during treatment. AA should be avoided in patients with serum potassium >5.0 mEq/L and in those with reduced renal function (baseline serum creatinine >2.0 mg/dl for women or >2.5 mg/dl for men, or an estimated GFR [2,8,14,15].…”
