AirWay Scope™ for Difficult Ventilation in a Patient With Epiglottic Cyst

Abstract: Epiglottic cysts often cause difficulty in airway management, with potential serious life-threatening complications.1 This case report describes a patient with an epiglottic cyst in whom mask ventilation became difficult after induction of anesthesia. Immediately, an AirWay Scope™ (TMAWS-S100; Pentax, Japan) was inserted orally, and the location of the epiglottis was clarified to assess the reason for difficulty with ventilation. This case demonstrates usefulness of the AirWay Scope in patients with epiglottic… Show more

“… 15 Moreover, infants quickly develop hypoxemia in a short period of apnea, and endotracheal tube insertion in younger pediatric patients also may be more difficult compared with older pediatric patients or adults despite an adequate view of the glottis. 16 Considering these points and previous cases in which the Pentax‐AWS® enabled successful intubation, 17 we used this device in the present case. This device allows for intubation to be performed quickly and for the epiglottis to be lifted to create space and provide a good field of view.…”

“…Laryngeal cystectomy was scheduled on POD 27. Based on the published reports, 6 , 7 we planned to manage the airway with a video laryngoscope (Pentax‐AWS®). After establishing intravenous access in advance, slow induction was started with sevoflurane and nitrous oxide to preserve spontaneous ventilation, and assisted ventilation with a mask was possible.…”

“…Regarding the examination of epiglottic cysts, video laryngoscopy can lift the epiglottis and large epiglottis cysts, thereby improving the patient’s ventilation and determining the cause of airway obstruction. 5 The potential complications of epiglottic cysts are infection and abscess. Early intervention depends on size and symptoms.…”

“…In this case, the patient’s slurred speech may have been due to the cyst blocking the airway and causing a change in pitch. Regarding the examination of epiglottic cysts, video laryngoscopy can lift the epiglottis and large epiglottis cysts, thereby improving the patient’s ventilation and determining the cause of airway obstruction 5. The potential complications of epiglottic cysts are infection and abscess.…”
