“…As described by others, a number of factors are required for judicious utilization of HEMS resources (e.g., medical oversight, cancellation by on-scene responders, strict utilization review). 30 Given the facts that rapid transport to trauma centers saves lives. 5 and that much of the U.S. population can reach Level I centers in a timely fashion only by HEMS, 4,31 appropriate use of helicopter resources is critical.…”

“…Ground EMS providers carry 20 different drugs but HEMS crews carry three times that number and can provide therapy such as blood transfusion and antibiotics for patients with open fractures [53]. Prehospital providers from some HEMS programs can provide advanced interventions such as tube thoracostomy; available evidence suggests that HEMS crew-placed chest tubes are as effective as, and no more likely to be associated with complications than, those placed in the hospital setting [54].…”

“…Especially in rural areas, the only prehospital care available may be BLS level [53]. It has been noted that since “HEMS brings a level of care to a trauma scene or small referring hospital that is over and above care rendered by an ALS ground ambulance,” many procedures such as intubation (even for patients transferred from referring hospitals) are deferred to the HEMS crew [53].…”

“…2,3 Helicopter evacuation of wounded soldiers was expanded in the Korean War and continues to be a main mode of transport in military conflicts to this day. 4 Several civilian models of helicopter transport were adopted in the early 1970s and have become so very imbedded in our EMS dogma that now, many cities have more than one air ambulance service available for their patients to get from the scene of a trauma to an accepting institution (primary transport) or even from the accepting institution to a tertiary care center (secondary transport). 5 The hypothesis of our study was that within our patient population, there exists a cohort of minimally injured patients not benefiting from helicopter transport that could safely be transferred from outlying institutions to our tertiary center.…”
