2The impact of rehabilitation on post-stroke motor recovery and its dependency on the patient's 3 chronicity remain unclear. The existence and regularity of a, so called, proportional recovery rule 4 across a range of functional deficits and therapies supports the notion that functional interventions 5 have little or no impact beyond spontaneous recovery rates in a 'critical window of recovery' 6 which lasts from 3 to 6 months post-stroke. In this meta-analysis, we apply a bootstrap analysis 7 method to assess the overall impact of a specific VR-based rehabilitation protocol for the upper 8 extremities on a homogeneous sample of 219 individuals with hemiparesis at various stages 9 post stroke. Our analysis uncovers a precise gradient of sensitivity to treatment that expands 10 more than one year beyond the limits of the so-called 'critical window of recovery'. These findings 11 redefine the limits of the so-called 'critical window of recovery' and suggest that stroke-derived 12 plasticity mechanisms do facilitate functional recovery even at the chronic and late chronic stage. 13

AIM: To evaluate the effects of a multimodal biofeedback training involving cycling augmented by Functional Electrical Stimulation (FES) and balance exercises on walking ability and motor recovery. DESIGN: Randomized controlled trial (NCT02439515). SETTING: Inpatient rehabilitation facility. POPULATION: Subacute stroke survivors (less than 6 months from the first event) with an age of up to 90 years old. METHODS: 68 participants were randomly allocated to an experimental group, performing 15 sessions of biofeedback FES-cycling training followed by 15 sessions of biofeedback balance training (20 minutes each) in addition to usual care (70 minutes), and a control group performing 30 sessions (90 minutes) of usual care. Participants were evaluated before training, after 15 sessions, after 30 sessions, and at 6-month follow-up through: gait speed (primary outcome), spatio-temporal gait parameters, Six Minute Walking Test, Functional Independence Measure, Motricity Index, Trunk Control Test, Berg Balance Scale, and Fall Efficacy Scale. RESULTS: Both groups significantly improved over time, but no group and interaction effects were found for any outcomes. The 73% of the experimental group achieved a clinically meaningful change in gait speed compared to the 38% of the control group (p-value=0.048).These percentages were even more unbalanced for patients with a moderate to severe gait impairment at baseline (91% versus 36%; p-value=0.008). Following stroke, the survival of neurons and their ability to re-establish connections is critical to functional recovery. This is strongly influenced by the balance between neuronal excitation and inhibition. In the acute phase of experimental stroke, lethal hyperexcitability can be attenuated by positive allosteric modulation of GABA A receptors (GABA A R). Conversely, in the late phase, negative allosteric modulation of GABA A R can correct the sub-optimal excitability and improves both sensory and motor recovery. Here, we hypothesized that octadecaneuropeptide (ODN), an endogenous allosteric modulator of the GABA A R synthesized by astrocytes, influences the outcome of ischemic brain tissue and subsequent functional recovery. We show that ODN boosts the excitability of cortical neurons, which make it deleterious in the acute phase of stroke. However, if delivered after day 3, ODN is safe and improves motor recovery over the following month in both young and aged mice. Furthermore, we bring evidence that during the sub-acute period after stroke, the repairing cortex can be treated with ODN by means of a single hydrogel deposit into the stroke cavity.
