Zusammenfassung Hintergrund Aufgrund des demographischen Wandels, der zunehmend auch die ältere Bevölkerung einschliessenden Entstigmatisierung psychischer Erkrankungen und nicht zuletzt dem erheblichen Zuwachs an spezifischen diagnostischen und therapeutischen Verfahren gewinnt das Fachgebiet der Alterspsychiatrie und Psychotherapie an Relevanz für die Grundversorgung. Entsprechend wichtig ist eine qualitativ hochwertige ärztliche Weiterbildung im Fachgebiet Alterspsychiatrie. Fragestellung Das Ziel der hier vorgestellten Übersichtsarbeit war es die Literatur zu medizindidaktischen Aspekten in der alterspsychiatrischen Weiterbildung zu sichten und aktuellen internationalen Entwicklungen in der kompetenz-basierten Weiterbildung gegenüberzustellen. Methode Für diese Übersichtsarbeit wurde die Scoping-Review-Methode nach Arksey und O’Malley verwendet. ErgebnisseDie initiale Suche ergab 913 Treffer. Nach Volltextsichtung wurden 20 Originalartikel für die Datenextraktion verwendet. Die Studieninhalte wurden drei Kategorien zugeordnet: Nachwuchsrekrutierung, Dauer und Struktur von Weiterbildungscurricula in der Alterspsychiatrie sowie alterspsychiatrische Lernziele und Kompetenzen. Mehrheitlich wurden Umfragen oder Expertenkonsensus als Methoden verwendet. Klinische Arbeitserfahrung mit alterspsychiatrischen Patient:innen und entsprechende medizindidaktische Betreuung von Assistenzärzt:innen war ein wichtiger Faktor für Interesse am Fachgebiet Alterspsychiatrie. Einzelne Evaluationsstudien weisen auf das Potential von digitalen Lehr- und Lernformaten und Simulationstrainings für alterspsychiatrische Inhalte. Insgesamt liegen bisher keine Studien in der Alterspsychiatrie mit explizitem Bezug zu kompetenz-basierten Weiterbildungskonzepten vor. Schlussfolgerungen Klinische Rotationen und Mentoring fördern das Interesse bei Assistenzärzt:innen am Fachgebiet Alterspsychiatrie. Weiterhin ist eine systematische Verankerung von klinischen Rotationen mit entsprechender strukturierter Wissens- und Fertigkeitsvermittlung in der alterspsychiatrischen Weiterbildung ein wichtiger Faktor. Medizindidaktische Begleitforschung im Fachgebiet Alterspsychiatrie mit Bezug zu Patientenparametern scheint ein sinnvoller nächster Schritt.

Group‐based exercise interventions for community‐dwelling older people in Southeast Asia: A systematic review

ObjectivesSoutheast Asia (SEA) is a rapidly ageing and a diversely populated region that requires strategies to maintain its populations' physical activity and sense of well‐being. While the benefits of group exercise programs are known, the characteristics and types of exercises in terms of their effectiveness for physical function and fidelity of the programs have yet to be defined within this population.MethodsOvid, MEDLINE, Scopus, PEDro (Physiotherapy Evidence Database), EBSCOHOST, Cochrane library and Open Grey databases were searched to identify relevant studies. Methodological quality was assessed using the PEDro Scale and the Newcastle Ottawa Scale (NOS). Meta‐analysis was undertaken when the same outcome measures were reported in a minimum of two studies with appropriate data. (PROSPERO: CRD42020177317).ResultsEleven studies with 900 participants were included, out of which 395 participants were allocated to group exercise programs and 383 completed the program. Culturally adapted Thai dance programs and multicomponent exercise programs were the most‐commonly reported group exercises. The Timed Up and Go test (TUG) and attendance rates were the most‐frequently reported outcomes. Meta‐analysis demonstrated significant improvement in physical function assessed using the Timed Up and Go test (Random effect model −1.27 s, 95% CI −1.65, −0.88, I2 = 74%). In two studies, adherence (81% and 94%) and dropout rates (4% and 19%) were reported.ConclusionsGroup‐based exercise programs in Southeast Asia consisting mostly of culturally adapted Thai dance programs and multicomponent exercise programs appear to have positive effects on physical function. However, better descriptions of fidelity, including adherence, are required in future studies.

Introduction The COVID-19 pandemic has necessitated the rapid expansion of telemedicine. However, there has been minimal coverage of telemedicine in traditional undergraduate medicine curricula. Telemedicine presents specific challenges in the geriatric population, including unfamiliarity with technology, cognitive and sensory barriers, inclusion of family and/or caregivers, multimorbidity, and a high degree of medical complexity. Methods We developed a workshop to allow rising third-year medical students to practice a telemedicine patient encounter while developing skills for assessing and communicating with geriatric patients. This 90-minute workshop consisted of an introductory didactic presentation and a standardized patient activity for small groups of two to five students. Students’ level of comfort with telemedicine for assessment of geriatric patients was evaluated with a pre- and postsurvey. Results Fifty-eight students participated in the workshop and completed the surveys (presurvey = 58, postsurvey = 40), with roughly half (52%) reporting prior experience with telemedicine. A 5-point Likert-type scale (1 = very uncomfortable, 5 = very comfortable ) was used. Students reported statistically significant increases in comfort using telemedicine (presurvey = 3.1, postsurvey = 3.9, p < .001) and using telemedicine for patients ≥65 years (presurvey = 2.8, postsurvey = 3.9, p < .001) after completing the workshop. Discussion Medical students’ comfort levels using telemedicine and caring for patients ages 65 and older with a telehealth visit improved after participating in this workshop. To help prepare students for telehealth practice in their future careers, educators should provide them with opportunities to practice and develop this critical skill set.
