‘Agglutination and flocculation’ of stem cells collected by apheresis due to cryofibrinogen

Abstract: Summary:Collection of peripheral stem cells by apheresis is a welldescribed process. Here, investigations concerning 'agglutination and flocculation' of stem cells collected from two patients are described. In both cases, cryoproteins were observed and cryofibrinogen was identified using highresolution two-dimensional electrophoresis. In one case, peripheral stem cells were collected after a second course of mobilization, and the cells were immediately washed at 371C before being frozen, allowing their use, de… Show more

“…Analysis of monoclonal gammopathies [45,46] and characterization of cryoproteins [47][48][49] using 2-DE has been shown to be useful in selected difficult clinical situations. Using 2-DE analyses and MS, we also recently identified a new peptide, called hSpa or CD5L [50].…”

“…Most frequently, the precipitates contain Igs and are therefore called cryoglobulins (reviewed in [46] [46]. In addition, proteins such as albumin [48] and fibrinogen [49] can also cryoprecipitate.…”

“…Our investigation revealed that compared to 2006, less ACD-A was used in products collected during 2007 and 2008. Although PBSC clotting can result from accumulation of different causes including plasma cryofibrinogens [9] or specific donor-related factors, insufficient anticoagulant was a reasonable contributing factor in these events. The ACD-A percentage in the two products described here was lower than 7%, which is much less than the average anticoagulant content for all three years.…”

“…Similar findings have been reported with autologous PBPCs and have been associated with cryofibrinogens. 2 Cryofibrinogens were not measured in our donor, but a blood sample examined after 2 days of refrigeration had no visible clumps. Based on our experience, we recommend the use of 5 percent albumin for processing of PBPCs if clumping is noticed when plasma is used.…”

“…Agglutination or clumping of hematopoietic progenitor cells has been known as a possible complication of the dimethyl sulfoxide (DMSO) cryopreservation process. 1,2 We encountered a 64‐year‐old man with Type 2 diabetes, coronary artery disease, status post–coronary artery bypass graft surgery, peripheral neuropathy, and a chronic ulcer of his foot, who donated peripheral blood progenitor cells (PBPCs) for his HLA‐matched sibling. His apheresis collections were uneventful.…”
