Prostate Cancer has assumed an important public health concern in Nigeria. Its incidence is anecdotally on the rise in our community and many prominent Nigerians are dying from it. The current study set out to ascertain the age adjusted prostate specific antigen (PSA) values indigenous to the local community and to confirm the relationship between PSA and age. In this study, enrollees were from a medical outreach carried out by a non-governmental agency in Benin City, Nigeria. The number of volunteers was 443 adult males. Eleven 11(2.48%) of this number were excluded from the study on account of their outlying PSA values. The mean age was 42.4years; the youngest volunteer being 18 years and the oldest, 86 years. The mean PSA value in the ≤40 year, 40-49, 50-59, 60-69, and ≥70year age categories were 1.77 ng/ml, 2.09 ng/ml, 1.99 ng/ml, 2.63 ng/ml, and 2.59 ng/ml respectively. The overall mean PSA was 2.21 ng/ml, whereas the median was 2.09ng/ml. Results showed positive but weak correlation between age and PSA (R = 0.161, P < 0.01). Nonetheless, significant increases in PSA with age were reported. The current study therefore provides a set of serum PSA values that are indigenous to the local environment, which could be used as cutoff threshold for performing prostatic biopsies.

Background Prostate cancer is the commonest cancer among men worldwide and serum prostate specific antigen (PSA) has remained the most commonly applied screening test for the disease till date. Current PSA test results guidelines in our population are informed by reference intervals derived from studies from Caucasians and other racial groups. With scanty data on PSA reference values from our local population, this study evaluated the serum PSA levels of apparently healthy Nigerian male subjects in whom prostate cancer and urinary tract infection have been excluded. Method This study had participants aged 40 to 70 years, with no lower urinary tract symptoms or other symptoms suggestive of prostate disease recruited from the male staff population of the University of Abuja and University of Abuja Teaching Hospital and the adjoining local community. They were physically examined, had prostate ultrasonography, urine analysis, and blood sample collected for PSA testing. Data collected was analyzed using Statistical Package for Social Science (SPSS) version 24. Result Of a total of 210 men who participated in the study, 191 eventually met the inclusion criteria. The average age was 52.9 years, ninety seven percent of them had heard of prostate cancer before now. The mean total PSA was 1.46 ng/mL (SD +/−1.55), while the reference interval was .23–5.60 ng/mL. The average prostate size was 41.8 mL (SD+/-20.11), and there was a positive correlation between the PSA and the prostate size (.418) as well as the age of the subjects (.446). There was no significant difference in the mean PSA value for those with or without family history of prostate cancer ( P=.979). Conclusion The reference range of PSA in Nigeria is higher than in other races, hence utilizing a local value in decision making would help to reduce unnecessary invasive procedures.

Background: In Nigeria, the diagnostic value of prostate-specific antigen (PSA) is a matter of debate. PSA levels are known to vary with population, environmental factors, and advancing age. Studies suggest age-specific reference intervals (ASRIs) of PSA value are more accurate than single cut-off PSA value. For ASRIs to be used effectively, reference intervals (RIs) must be fully evaluated. Aim: We determine ASRIs in a Nigerian population. Materials and Methods: The study was carried out from January 2016 to January 2019 among 660 adult Nigerian men aged 30–86 years old in Enugu State. Participants completed questionnaire demographics and previous screening. Age group was the indicator. Among them, a total 24 (3.6%) were excluded. Data from 636 (96.4%) men were analyzed for ASRIs. Estimation of PSA was done as per the International Federation of Clinical Chemistry Guideline. Spearman correlation was used to identify correlates P values < 0.05 which was considered significant. Results: The mean age group was 49.6 ± 10.2 years. ASRIs using 95th percentile, and PSA values in each 10 years groups were 0–1.94 ng/ml (median 0.22), 0–2.52 ng/ml (median 0.42), 0–3.52 ng/ml (median 1.06), 0–4.8 ng/ml (median 2.1), 0–6.95 ng/ml (median 4.1), and 0–5.6 ng/ml (median 2.4), for age groups 30–39, 40–49, 50–59, 60–69, 70–79, and ≥80 years, respectively. There was positive correlation between PSA and age (r = 0.9915, P < 0.0001). Low income and educational background were more prevalent among the study group. Conclusion: Our study provided the ASRIs in our environment but higher than single cut-off value. The data recommended PSA values should be characterized by age and ethnicity.
