Öz Amaç: Çalışmada isotretinoin tedavisi başlanan ve kümülatif doza ulaşan hastalarda tedavi süresince ilaç kullanımı ile dikkat eksikliği arasındaki bir ilişkinin olup olmadığının tanımlanarak literatüre katkıda bulunması amaçlanmıştır. Gereç ve Yöntem: Akne vulgaris tanısıyla isotretinoin tedavisi başlanan ve kümülatif doza ulaşan 16-40 yaş arası 100 hasta çalışmaya alınmıştır. Hastaların tedavinin başlangıçında, tedavinin 3. ayında ve 6. ayında Stroop testi ile dikkat ölçümleri (SCWT) değerlendirilmiştir. Bulgular: Hastalara ilk başvurularında, tedavinin 3. ayında ve 6. ayında Stroop testi uygulanarak elde edilen SCWT ölçümlerinin zamana göre değişip değişmediğini belirlemek için tekrarlı ölçüm ANOVA analizi yapılmıştır. Stroop testine ait tüm ölçümlerin zamana göre değişimi istatistiksel olarak anlamlıdır. Her beş ölçüme ait sürelerin zamana göre anlamlı olarak azaldığı tespit edilmiştir. Sonuç: İsotretionin başlanan kümülatif doza ulaşan hastaların tedavi başlangıcında, tedavinin 3. ayında ve 6. ayında yapılan Stroop testi sonuçları, isotretinoin tedavisi ile dikkat eksikliği arasında bir ilişkinin olmadığını testlerin okunma sürelerinde görülen anlamlı azalmanın, katılımcıların Stroop testini öğrenme etkisinden kaynaklanmış olabileceği düşünülmüştür. Bu prospektif çalışma, isotretinoin kullanımı ile dikkat eksikliği arasında nedensel bir bağlantı olmadığını göstermektedir.

ÖZET Amaç: Akne vulgaris (AV) hastalarında izotretinoin tedavisinin uyku ve yaşam kalitesine olan etkisini incelemeyi amaçladık. Gereç ve Yöntem: Çalışmaya dermatoloji polikliniğine başvuran, AV tanısıyla izotretinoin tedavisi başlanan 66 hasta ve 43 sağlıklı kontrol grubu dahil edildi. AV hastalarına tedaviden önce ve iki ay sonra Pittsburgh Uyku Kalite İndeksi (PUKİ), kısa form-36 testleri uygulandı ve Global Akne Derecelendirme Sistemi ile akne şiddeti hesaplandı. Sonuçlar sağlıklı kontrol grubu ile karşılaştırıldı. Bulgular: Hasta grubunun tedavi öncesi fiziksel sağlık skorları kontrol grubuna göre anlamlı yüksek bulundu. Akneli hastalarda izotretinoin tedavisi öncesi ve sonrası fiziksel, mental sağlık skorları açısından anlamlı fark saptanmadı. Tedavi öncesi grup ve kontrol grubu arasında toplam PUKİ puanları açısından anlamlı fark saptanmazken; uyku süresi puanları tedavi öncesi grupta anlamlı düşüktü. Akneli hastalarda tedavi öncesi ve sonrası toplam PUKİ puanları ve alt bileşenleri açısından anlamlı fark saptanmadı. Akneli hastalarda tedavi öncesi ve sonrası toplam PUKİ puanları ile fiziksel, mental sağlık skorları arasında anlamlı negatif korelasyon saptandı. Sonuç: Çalışmamızın bulguları izotretinoin tedavisinin uyku kalitesini etkilemediğini ortaya koymaktadır. Akneli hastalarda uyku kalitesinin bozulması yaşam kalitesini olumsuz etkilemektedir. İzotretinoinin uyku kalitesi üzerine etkisini değerlendiren daha kapsamlı ve ileri çalışmalara ihtiyaç vardır. ABSTRACT Objective:We aimed to investigate the effect of isotretinoin treatment on sleep and quality of life in patients with AV. Methods: 109 total patients who identified as 66 AV patients attended the dermatology outpatient clinic and started isotretinoin treatment, and 43 healthy controls, were enrolled in the study. Pittsburgh Sleep Quality Index (PSQI), short form-36 (SF-36), and Global Acne Grading System (GAGS) were administered to all patients twice; before and two months after the treatment. The results were compared with the healthy control group. Results: The pretreatment physical health scores of the patients were significantly higher than the healthy controls. No significant differences were observed between before-after isotretinoin treatment regarding physical and mental health scores in AV patients. While there were no significant differences between the pretreatment and healthy control group in terms of total PSQI scores; the scores of sleep duration were significantly lower in the pretreatment group. No significant differences were observed between before-after isotretinoin treatment regarding total PSQI and subcomponent scores in AV patients. There was a statistically significant negative correlation between pre-and post-treatment total PSQI scores and physical, mental health scores in patients with AV. Conclusion: Results of the present study suggest that isotretinoin therapy does not affect the sleep quality in patients with AV.
